Managing UDS Information

The UDS tab appears only when the Display person UDS fields option is enabled in Enterprise Preferences, UDS. On the tab, Community Health Centers can enter information for a person or patient that is used on the Uniform Data System (UDS) report, and Indian Health Systems can enter information needed for their reports. In Enterprise Preferences, each UDS field can be set up to be required for all persons and patients when a demographic record is created. In Practice Preferences, each UDS field can be set up to be required either for all persons and patients when an appointment is scheduled, or for all patients when a chart is created. Relationship Information

In the Relationship section of the Relations/Roles tab, you can enter other people that are associated with a person or patient in the practice. For example, a patient may have relationships to a guarantor, an insurance policyholder, an emergency contact, and a head of household. Relationship information is stored at the system level; therefore, all practices in all enterprises share the same information for a person or patient. Birth Mother's Full Name

The Birth Mother's Full Name section appears on the Relations/Roles tab only when enabled in the following areas: Enterprise Preferences, General: Select the Birth Mother’s full name on Relations Tab check box to display the same check box in Practice Preferences for all practices. If not selected, the check box does not appear in Practice Preferences for any practice. Contact Information

In the Contact section of the Relations/Roles tab, you can enter the person designated as the primary contact for a patient. The information is saved for established patients with an existing chart, and for new patients when the chart is created. Contact information is stored at the practice level; therefore, each practice can enter and maintain different information for a patient. Support Role Information

In the Support Roles section of the Relations/Roles tab, you can enter people that play an important role in the support of a person or patient in the practice. A support role can be a caregiver, an emergency contact, or a next of kin. Support roles and relationships are stored as separate demographic records in the database. Relationships are assigned a person number, and they are part of the Master Person Index (MPI). Relationships can be found from the People Lookup and Patient Lookup windows. Support roles are not assigned a person number, and they are not part of the MPI. Support roles cannot be found from the lookup windows. If a person is both a relationship and a support role for a patient in the practice, updating the demographic record for one does not update the demographic record for the other. Guarantor Information

In the Guarantor section of the Relations/Roles tab, you can enter the person designated as the chart-level guarantor for a patient. This person defaults as the guarantor on all new encounters created for the patient, and is financially responsible for any outstanding unpaid patient balances. The information is saved for established patients with an existing chart, and for new patients when the chart is created. Guarantor information is stored at the practice level; therefore, each practice can enter and maintain different information for a patient.
