Add or Modify Uniform Data System (UDS) Information

UDS Information

In the UDS section of the UDS tab, you can enter information that is not part of the standard demographics record for a person or patient. This is typically used by Community Health Centers to capture information such as homeless status and migrant worker status. It is also used by Indian Health Systems to capture information such as tribal affiliation and blood quantum. UDS information is stored at the system level; therefore, all practices in all enterprises share the same information for a person or patient. Add or Modify UDS Information

Open the Person Information or Patient Information window for the desired person or patient. Select the UDS tab. In each of the following UDS fields, select the appropriate item from the list: Homeless Status Migrant Worker Status Language Barrier Public Housing Primary Care School Based Health Center Veteran Status: Defaults to Not Collected Yet and can be changed if needed. Add or Modify Family Information in UDS Tab

To enter family information, you must ensure that a head of household is selected for the patient. Note: You can modify family information only if it has not been used. Specifically, if family size and income have been used to calculate automatic adjustments on an encounter, that information cannot be modified. Only the Verify Date and Effective/Expiration Dates can be modified. Open the Person Information or Patient Information window for the desired person or patient. Select the UDS tab. In Family Information, select the open menu icon, and then select New or Open. In the Family Info window, enter or update the details as needed. Select Income Cycle. If the head of household refuses to provide income information, select the Refused to Report checkbox. Select OK. Parent topic: Family Information

Information Needed for the UDS Report

Although some data required for the UDS Report is more readily (or only) available from a general ledger or other external system, much of the data can be reported directly from NextGen® Enterprise PM. Several demographic fields required by Health Centers for the UDS report are available on the UDS tab of the Add/Modify Patient Information window. Additional fields are available for use by IHS practices. To display the UDS tab, enable the Display person UDS fields check box in Enterprise Preferences > UDS.
