window for the desired person or patient.
Select the
UDS
tab.
In each of the following
UDS
fields, select the appropriate item from the list:
Homeless Status
Migrant Worker Status
Language Barrier
Public Housing Primary Care
School Based Health Center
Veteran Status: Defaults to
Not Collected Yet
and can be changed if needed.
In each of the following
IHS
(Indian Health Services) fields, select the appropriate item from the list:
Tribal Affiliation
Blood Quantum
IHS Eligibility Status
Classification Beneficiary
Descendancy
In each of the following
Demographics
fields, select the appropriate item from the list:
Race
Ethnicity
Current Gender
Gender Identity
Sexual Orientation
Preferred Pronoun
Note:
The above fields appear on either the
Demographics
tab or the
UDS
tab, depending on settings in Enterprise Preferences, General.
In the
Primary Medical Coverage
field, do one of the following:
If the patient has insurance, then select their
Primary Payer
from the list.
If the patient does not have insurance, then select the
Self Pay check
box.
When selected, the
Primary Medical Coverage
field becomes disabled.
To indicate the patient has given consent for treatment, select the
Consent To Treat
check box.
When selected, the
Consent To Treat Date
field becomes enabled.
In the
Consent to Treat Date
field, enter the date the patient's consent was given.
The
Consent to Treat
check box must be selected.
To save changes and close the window, select
OK
.

Add or Modify UDS Information
Open the Person Information or Patient Information window for the desired person or patient. Select the UDS tab. UDS Fields
In NextGen® Enterprise PM, additional fields can be enabled to capture information on patients that is required for the UDS report. When the fields are enabled, a UDS tab appears on the Add Patient Information and Modify Patient Information windows. Add Patient Information In each UDS field, you can then select the appropriate value for each patient. The values available for selection are from the corresponding master list table in File Maintenance as outlined below. Chart Practice Preferences Fields
The following table describes the fields located in Chart Practice Preferences. Chart Field Description User-Defined Names 1-8 Enter a label for each of the user-defined fields that you want to display on the patient's chart. You can use these fields at any time or you can make them required entries. To make one of these fields required at chart creation, select the corresponding Required check box. These user-defined fields represent information that remains constant from encounter to encounter, for example, "organ donor." When the User-Defined Name is created and selected as required, the information from the user-defined fields displays on the patient's chart and the Chart tab, as well as on the Chart/Ins tab of the Appointment Book. Enabling and Requiring UDS Fields
Enabling UDS Fields When UDS fields are enabled, the UDS tab displays on the Add/Modify Patient Information window in NextGen® Enterprise EHR and NextGen® Enterprise PM. Users can then access the tab to select the appropriate UDS values for each person/patient. UDS fields are enabled at the enterprise level. Therefore, if enabled, the UDS tab displays on the Add/Modify Patient Information window in all practices within the enterprise. Requiring UDS Fields UDS fields can be set up to be required at either the enterprise level or at the practice level. If required at the enterprise level, users in all practices in the enterprise must enter data in the UDS fields for all persons (globes) and all patients (charts) when the demographic record is created for a person/patient. If required at the practice level, there are two points at which users in the practice must enter data in the UDS fields; either when an appointment is scheduled for a person/patient, or when the chart is created for a
