Modify the Billing ICD List Data Entry Fields

You can modify selected entries in the billing ICD list using the billing ICD list data entry fields.
You can modify the billing ICD list data entry fields for an ICD selected in the billing ICD list.

  1. Open Billing ICD List.
  2. In the Billing ICD List data entry area, verify the information entered in the Encounter Date, Code, and Description fields.
  3. To select the onset date, select the Onset Date field and use the calendar.
  4. Select the Diagnosed Date field and use the calendar to select the diagnosed date.
  5. Select the Resolved Date field and use the calendar to select the correct resolved date.
  6. If you receive an application message for the Diagnosed Date or the Resolved Date, select OK and select a new date or create a encounter.
  7. Select or clear the Chronic check box to specify whether the diagnosis is chronic.
  8. Select or clear the Recorded Elsewhere check box to indicate whether the diagnosis is recorded elsewhere.
  9. Check that the information entered in the Source field is correct.
    This information is not directly editable.
  10. Select the Status arrow and select a Status.
    If the required choice is unavailable, contact your System Administrator.
  11. Select the Severity arrow and select a Severity.
    If the required choice is unavailable contact your System Administrator.
  12. Select the Verification Status arrow, and select a Verification Status.
    Existing problems have a blank Verification Status that can be manually updated. Changes in Verification Status is tracked in the Encounter History.
  13. Select a Provider and Location, if different than the default selection.
  14. Select the Problem Type list and select a problem type.
    The Problem Type list displays a list of SNOMED® problem type codes and their corresponding descriptions. For new patient diagnoses, the default Problem Type value is 282291009– Diagnosis Interpretation (Observable Entity). For historical patient problems and diagnoses, the Problem Type is blank and can be manually updated.
    The external codes imported through CCDA that are not in the NextGen® Enterprise EHR are displayed in the billing ICD list display area. To enable the columns that display problem type data, select Problem Type Description and Problem Type Code in the Preferences button on the data display area toolbar.
  15. Add any appropriate notes in the Note field.
  16. Do one of the following:
    • Select Accept to add the diagnosis/ICD code and its details to the patient's chart.
    • Select Cancel to rest all the data entry fields to their default settings.
