Patient's Problem List

You can launch the diagnosis search window from a Problem List Pop-Up trigger to display only the diagnoses and ICD codes that have been assigned to the patient. You can then select an ICD code from the list. For example, you may want to associate an existing ICD code with a procedure for billing purposes. Note: In this mode, you cannot search All Diagnoses, Favorites, or Categories; use ICD Legacy Mapping; or view, add, or select ICD synonyms. Enter your search criterion in the text box at the top of the diagnosis search window. You can enter all or part of the clinical description or ICD code or description. Select Search. The search results grid displays the patient problems that match your search criterion. To close the diagnosis search window without making a selection, select Cancel. To select an ICD code, double-click the result row you want, or select the row and select Select. The ICD code is automatically entered in the template field assigned that value. Search All Diagnosis

Enter your search criterion in the text box at the top of the diagnosis search window. You can enter all or part of the clinical description, ICD code, or billing description. You can search for both ICD-9 and ICD-10 codes. You can enter a higher-level ICD code to display all of the associated lower-level ICD codes. Note: The search criterion may be populated from the trigger. You can change it, if necessary. If you search for ICD code 250.00, the search results include codes such as 250.01 or 250.02. Select All Diagnoses (default) in the left pane. Select Search, or select the Enter key. The search results grid displays the results that match your search criterion. Any results that match a diagnosis already on the patient’s diagnosis list display at the top followed by any results that match the provider’s most frequently assigned list. ICD Legacy Mapping

ICD Legacy Mapping enables you to view ICD-10 General Equivalency Mapping (GEM) codes for an ICD-9 code. These code mappings are drawn from the ICD-9-to-ICD-10 xref table supplied by Health Language, Inc. (HLI), a third party content provider. The diagnosis search window may be set up to launch in the ICD Legacy Mapping mode, or you can select an option for this mode on the window. Enter the ICD-9 code in the text box at the top of the diagnosis search window. Select ICD Legacy Mapping in the left pane. Select Search, or select the Enter key. The search results grid displays each unique ICD-10 code mapped to the ICD-9 code, which is shown in the Legacy Icd Code column. Note: The results show only the display term description of each unique ICD code, not every synonym for it. To view ICD code synonyms, see step 4. Set up Diagnosis Search Result Columns

You can specify the columns, column order, and the type of description to display for search results in the diagnosis search window. The system retains the last display settings the next time the diagnosis search window launches. Select Preferences on the diagnosis search window. The Columns Configuration menu opens. To specify which columns to display, do one of the following: Select each column you want to display from the menu. A check mark appears to the left of the selected column to indicate that it is set to show in the grid. To hide the column and remove the check mark, you can select the column again. Note: You may find it helpful to display the ICD code and clinical description in separate columns. If the ICD code is in its own column, you can sort the column to view all of the lower-level ICD codes for a higher-level code you enter. Diagnosis Categories Search

From the Master Files - System list, select Diagnosis Categories. The Diagnosis Categories List window opens. To add a new diagnosis category, select Open Menu , then select New. To search for an existing diagnosis category to modify, follow the steps below: In the List Search field, enter what you want to search for. The matching results are listed. Note: You can enter an asterisk ( * ) to see all the records. Double-click the diagnosis category you want to view. The Add/Modify Diagnosis Category Information window opens. Enter a name for the diagnosis category in the Category field. Enter a four-digit abbreviation for the diagnosis category in the Short Description field. To add an ICD code and description select Open Folder . The ICD Codes window opens. To add a code to the category, select the code in the Available list, then select the Include . The code moves from the Available list to the Included list. If necessary, select either 09 or 10 in the Show field to display only ICD9

Select Diagnosis and Admitting Diagnosis for a Procedure

You can either enter or search and select a diagnosis for a patient's procedure. You must select at least one diagnosis for the patient. To enter up to 12 diagnoses for a procedure, you must select Allow twelve diagnoses under the EHR tab in thePractice Preferences window in File Maintenance. Open NextGen® Enterprise EHR. Open Procedures window in Procedures modules. In Diagnosis field, enter a diagnosis. If a diagnosis code does not exist in the patient's chart, a message appears confirming that add the diagnosis to the patient's chart. To add the diagnosis, select Yes. To continue adding the procedure without adding the diagnosis to the patient's chart, select No. To select a Diagnosis, select the ellipsis button. The search window opens. Search Select the preference icon and select the columns to display. Search results The selected columns appear in the search window.
