Viewing Patient Confidential History

On the Title X 2.0 template, the Confidential History panel displays a patient's contraception and reproductive-life planning information based on the following clinical templates: Title X Confidential History panel Field Description Include all confidential information in the social history document with patient permission You can configure confidential information to be added to the social history document. Birth control method at beginning of visit Enables you to document the finding on the OBGYN Synopsis or Confidential Patient Information template. Reason no birth control method at beginning of visit Enables you to document the finding on the OBGYN Synopsis or Confidential Patient Information template. Would you like to be pregnant in the next year? Enables you to document the finding on the Confidential - Sexual Practices/STIs template. View Historical Tobacco Usage

On the Social History- Tobacco template, expand the Historical Use panel. If the client has encounters entered on a NextGen® Adaptive Content Engine release prior to version 7.9.1, click the Click here to see tobacco history prior to 7.9.1 link. The Tobacco Use History template opens. Click Save & Close to return to the Social History - Tobacco template to review the client's tobacco history that was documented in versions later than 7.9.1. Add Family/Social History

Select Fam/Soc Hx. Fam/Soc Hx button under Past Medical History section A picklist opens for you to select the type of history you want to document. Select the history, and then select OK. The Social History template opens to the page you selected. You can select other social histories from the left navigation pane. You can document or review detailed information about a patient's current or past tobacco use, including smoking and vaping, on the Social History - Tobacco template. You can also record a patient's tobacco cessation efforts and exposure to passive tobacco smoke. social history - tobacco You can open the Social History - Tobacco template from the Social History panel on the Histories - OPH template.
