Create a Treatment Plan for the Patient
Treatment plans are set up on the PT Treatment Setup template. The PT Treatment Setup template displays the Menu and the Treatment Plan panels. The Menu panel displays a list of treatment categories in the left pane. The Treatment Plan panel grid displays the treatment options previously selected for the patient. Treatment plans configured on the PT Treatment Setup template appear in the Performed Today section in the Treatment Panel and carry forward to future encounters. On the Objective template, click the Treatment button in the Treatment panel to launch the PT Treatment Setup template. Pt Treatment Setup Click the Body region field and select from the list. The Menu panel grid displays all treatment options for the selected region. Select a treatment category from the left pane. The Menu panel grid refreshes to display that category’s treatment options for the region. Click the rows in the Menu panel grid that display a treatment you wish to include in the patient’s plan. Document Treatments Performed Today
The Performed Today section is used to document what treatments performed during the treatment encounter and enter additional information. Treatments, CPT and Time (mins) values carry forward to future encounters where they can be modified or updated as needed. Treatment On the Objective template, navigate to the Treatment panel. In the Performed Today section, complete treatment details performed. Click the Weight field and use the numeric keypad to enter the weight used when performing the exercise/treatment, if applicable. Click the Sets field to enter the number of sets of the exercise/treatment performed, if applicable. Click the Reps field to enter the number of repetitions performed in each set of the exercise/treatment. Click the Exertion field and select the exertion level when the exercise/treatment was being performed from the picklist. If the time in minutes spent performing the exercise/treatment is different from the time set up in the treatment plan, click the Time field

Physical Therapy Treatment Panel
The Treatment panel enables you to document treatments performed during an encounter. Information documented here is also used for billing purposes. The Treatment grid displays details about the treatment plans. The fields in the Performed Today section below the grid function as a worksheet for documentation of treatment activities performed. Note: Only those treatments detailed in a row with the Performed check box selected are billed. Access Home Page - Physical Therapy
Before you begin When you open the Home Page - Physical Therapy template, you must select the Specialty and Visit Type to access the framework template set for Physical Therapy. Open NextGen® Enterprise EHR. Click the Medical Records tab. In the Show section, select the All option to display the list of templates. Select Home Page - Physical Therpay. Physical Therapy Home Page Template Details
The practice can customize the panels displayed on the Home Page - Physical Therapy template. The following table describes the panels on the template. Panel Description Diagnosis Lists all the assessments to be included in orders and billing. You can view billing assessments for the current encounter, including ICD codes. Goals Displays patient's therapy goals documented by the provider in the Therapy Goals template. Treatment History Displays the treatment hisory details related to the patient. Historical PE Displays the Historical PE details. Treatment Log Displays the patient's treatement details by encounter. Lists the patient's encounter diagnosis, treating assessments, and a history of treatments in a flowsheet format, organized by the encounter date. Allergies Lists the allergies associated with a patient. Medications Lists the patient's current medications. Histories Template
Patient history information is documented on the Histories template for a new patient, and reviewed and updated for an existing patient. Histories Patient's Problem List You can manage a patient's problems. Patient's Medical History The Medical/Surgical/Interim panel enables you to document the medical or surgical history of the patient. Patient's Diagnostic Tests and Results You can document a patient's diagnostic tests and the associated results. Patient's Family History You can document the family history of the patient in the Family panel on the Histories template.
