Assessment/Plan Panel

The Assessment/Plan panel provides a summary of the assessments and plan information for the patient for the current encounter. In Physical Therapy, assessments for the encounter are selected at the beginning of the encounter, therefore the Assessments/Plan panel grid displays the previously selected assessment(s). Patient Goals

The Goals panel displays the therapy goals documented by the provider on the Therapy Goals template. Any additions to the grid must be made on the Therapy Goals template. Document Treatments Performed Today

The Performed Today section is used to document what treatments performed during the treatment encounter and enter additional information. Treatments, CPT and Time (mins) values carry forward to future encounters where they can be modified or updated as needed. Treatment On the Objective template, navigate to the Treatment panel. In the Performed Today section, complete treatment details performed. Click the Weight field and use the numeric keypad to enter the weight used when performing the exercise/treatment, if applicable. Click the Sets field to enter the number of sets of the exercise/treatment performed, if applicable. Click the Reps field to enter the number of repetitions performed in each set of the exercise/treatment. Click the Exertion field and select the exertion level when the exercise/treatment was being performed from the picklist. Create a Treatment Plan for the Patient

Treatment plans are set up on the PT Treatment Setup template. The PT Treatment Setup template displays the Menu and the Treatment Plan panels. The Menu panel displays a list of treatment categories in the left pane. The Treatment Plan panel grid displays the treatment options previously selected for the patient. Treatment plans configured on the PT Treatment Setup template appear in the Performed Today section in the Treatment Panel and carry forward to future encounters. On the Objective template, click the Treatment button in the Treatment panel to launch the PT Treatment Setup template. Pt Treatment Setup Click the Body region field and select from the list. The Menu panel grid displays all treatment options for the selected region. Select a treatment category from the left pane. The Menu panel grid refreshes to display that category’s treatment options for the region. Click the rows in the Menu panel grid that display a treatment you wish to include in the patient’s plan. Treatment Log Template

The Treatment Log - Physical Therapy template enables providers to review the patient’s encounter diagnoses, treating assessments, and a history of treatments in a flowsheet format, organized by encounter date. In addition, documents for orders, work notes and patient encounters can be generated here. The Diagnosis panel on the Treatment Log template is read-only. The Diagnosis panel displays the Encounter Diagnosis and Treating Assessment sections. The Treatment Log panel displays a read-only view of treatments by encounter.
