Screen for Future Fall Risk
You can document the patient's risk for falls. Open NextGen® Enterprise EHR. To open the Social History – Lifestyle template, do one of the following: On the Histories template, on the Social panel, select Lifestyle , and select Add/Edit. In the History Summary panel on Intake, select Lifestyle from Social, and select Add/Edit. The Social History – Lifestyle template opens. In the Home Environment/Safety section, in Falls in the last year?, select the applicable option. If you select Yes, enter the number of falls. In Did the fall(s) result in injury?, select the applicable option. If you select Yes, then enter information about any injuries resulting from falls in the Details field. In Is the patient at risk for falls?, select the applicable option. Select the Fall Risk Plan link to access the Fall Risk section of the Health Promotion Plan template, if needed. Patient's Social History
You can document a patient's social history. You can open the individual social history template or the Adult Social History template to document the social history information of a patient. The Adult Social History template includes the existing social history templates arranged in a one-page panel format. Each panel includes the data fields from the respective social history template. You can choose to document the social history information of the patient on individual social history templates or on the Adult Social History template. Health Promotion Plan
You can document a patient's health promotion plan on the Health Promotion Plan template. You can document the following details on the Health Promotion Plan template. BMI plan Depression plan Hypertension plan Pain management plan Fall risk Screening tools and follow-up care plans Functional and Cognitive Assessment Status Health Promotion Plan You can select the information icon to open the Information Display window, which displays all the details required for successful quality reporting. For adult patients, you can document the diet, physical activity, and referral details. For patients younger than 18 years, you must document the diet and physical activity details. The Exceptions link enables you to manage the exceptions on the Exceptions/Measures template. This link appears in all panels except Functional and Cognitive Assessment Status. Document Patient's Fall Risk
You can document a patient's risk of falling. Open NextGen® Enterprise EHR. Open the Intake template. Expand the Vital Signs panel, and then select the Health Promotion Plan link. The Health Promotion Plan template opens. Expand the Fall Risk panel. In Falls in the last year?, select the applicable option. If you select Yes, enter the number of falls. In Did the fall(s) result in injury?, select the applicable option. If you select Yes, then enter information about any injuries resulting from falls in the Details field. In Is the patient at risk for falls?, select the applicable option. To manage exceptions and view measure details on the Exceptions/Measures template, select Exceptions. In the Assistive devices field, enter the follow-up plan of care information. If the recommendations include an exercise program, enter the details in the Balance, strength, and gait training field. Document Risk Assessment
Open a risk assessment template, and expand the Risk Assessment panel. To document a potential for suicide and homicide risk, select one or more of the following links: Suicidal/Homicidal Risk Safety Risk Columbia Suicidal Severity Rating Scale (C-SSRS) SAFE-T Protocol with C-SSRS (Columbia Risk and Protective Factors) Note: Only one of the C-SSRS template appears based on the configuration in the BH Foundation Configuration template.
