Document the Patient's Status

You can document the status of a patient on the Social History - Status template. Open NextGen® Enterprise EHR. To open the Social History - Status template, do one of the following: On the Histories template, expand Social, select Statuses, and then select Add/Edit. In the History Summary panel on the Intake template, select Social, select General, and then select Add/Edit. Social History - Status In the Demographics section, document the patient's race, ethnicity, and preferred language, the language they speak at home, and their dominant hand. In the Employment section, do the following: Select Add, and on the Employment/Occupation – [New Record] template, enter the patient's occupation, employer information, employment status, restrictions, and other details. Select Save, and then close the template. Select the occupational hazards or enter the hazards manually. You can enter up to two hazards. Open the Histories template. In the Social panel, select Lifestyle and click Add/Edit. The Social History – Lifestyle template opens. In the Home Environment/Safety section, in Falls in the last year?, select the applicable option. If you click Yes, enter the number of falls. In Did the fall(s) result in injury?, select the applicable option. If you click Yes, then enter information about any injuries resulting from falls in the Details field. In Is the patient at risk for falls?, select the applicable option. Click the Fall Risk Plan link to access the Fall Risk section of the Health Promotion Plan template, if needed. To open CQM - Falls Risk , click the Quality button. Note: You can view the information about Falls Risk clinical quality measure from CQM - Falls Risk . Click Save & Close. Document Patient's Fall Risk

You can document a patient's risk of falling. Open NextGen® Enterprise EHR. Open the Intake template. Expand the Vital Signs panel, and then select the Health Promotion Plan link. The Health Promotion Plan template opens. Expand the Fall Risk panel. In Falls in the last year?, select the applicable option. If you select Yes, enter the number of falls. In Did the fall(s) result in injury?, select the applicable option. If you select Yes, then enter information about any injuries resulting from falls in the Details field. In Is the patient at risk for falls?, select the applicable option. To manage exceptions and view measure details on the Exceptions/Measures template, select Exceptions. In the Assistive devices field, enter the follow-up plan of care information. If the recommendations include an exercise program, enter the details in the Balance, strength, and gait training field. Add Social History

You can add the social history of a patient in the History Summary panel on the Intake template and in the Social panel on the Histories template. Open NextGen® Enterprise EHR. To document the social history details, do one of the following: In the History Summary panel on the Intake template, select Social, select the relevant option in the list and Add/Edit. On the Histories template, expand the Social panel, select the relevant option, and then select Add/Edit. The social history template opens. Document the reported history information. Select Save & Close. Categories in Social History You can use different sections in the Social history to document the history of a patient. Patient's Tobacco Use Information You can document the patient's current or past tobacco use information. Document Registry-Based Exceptions

You can document a registry based exception. Open NextGen® Enterprise EHR. To open the Exception/Measures template, do one of the following: In the navigation bar, select the Exceptions/Measures link. On the Finalize template in the CPT II Claims Based Measure Reporting panel, select the Exceptions/Measures link. Exceptions/Measures For Display, select one of the following: Provider Measures Exceptions (current template) Exceptions (all) Select a measure in the grid. The details of the selected measure appear in the Measure Details and Exception Management sections. If applicable, select the exception class for the measure. Note: If the measure exists in multiple exception class records, a window appears, where you can select the exception class. If Clinical Quality Measures (CQM) has single exception class, an exception class record is automatically selected. The details for exception class appear in the Exception Management section. Viewing Exceptions and Measures

You can manage exceptions and view measure details on the Exceptions/Measures template. You can open the Exceptions/Measures template from the navigation bar or from the CPT II Claims Based Reporting panel on the Finalize template.
