Record the Patient's Derm Risk Factors

Open NextGen® Enterprise EHR and select a Established patient or New patient as appropriate. Create a patient encounter and open the Intake template. Select a Specialty of Dermatology and a Visit Type of Office Visit. The Derm Risk Factors Config link is available on the patient header of every dermatology template Click the Config icon. The Derm Risk Factors Config template opens. If the patient does not have any risk factor, select the No risk indicators check box. In the Tobacco section, document the smoking status of the patient as follows: Click Tobacco Usage to document the tobacco consumption details such as tobacco type and usage per day on the Social - History Tobacco template. Click Tobacco cessation discussed to document smoking cessation efforts and passive smoke exposure on the Tobacco Cessation Discussed template. Select Yes, No, or Unknown for other risk factors. The default setting is Unknown. The risk factors for Melanoma (MM), Diabetes (DM), Basal cell carcinoma (BCC)

Documenting Patient Encounters for Dermatology Visits

The Dermatology User Guide forNextGen®Adaptive Content Engine provides the guidelines for conducting a comprehensive patient encounter in the Dermatology specialty. During Dermatology patient encounters, you can use various features that are designed to streamline your workflows and improve your data captures. The intake staff checks in the patient on the Intake and Histories templates. If required, the intake staff can review the patient's details documented in the previous encounters on the Home Page template. The provider can choose to review the patient's historical information documented in the previous encounters on the Home Page template or review the Intake Note document generated by the intake staff. After reviewing the information, the provider documents the patient examination on the GET and SOAP templates. The provider completes the visit and selects the E&M code on the Finalize template. Viewing Risk Factors

You can view details about patients' risk factors to help you to evaluate their health condition. The risk indicators for Tobacco use (TOB), Hypertension (HTN), Diabetes (DM), and Coronary artery disease (CAD) are available for documentation in NextGen® Adaptive Content Engine. Risk factors You can use the Risk Factors Config template to indicate a patient's current health conditions. You cannot consider a patient's health at high risk by looking at these conditions or you can view the risk indicator icons without opening the template. If you want to indicate that the patient is at high risk for developing a certain condition, you can document such details on the Care Guidelines template. You can use the risk indicators to review the risk factors without opening the Risk Factors Config template. Document the Risk Factors You can document the risk factors for patients. Click the Configure icon on the top corner of the template.The Oph Risk Factors Config template opens. oph risk factors config If the patient has no risk conditions, select the No risk indicators check box and then click Save & Close. Select Yes, No, or Unknown for the following risk factors. You can identify a patient's risk factors by viewing the risk indicators associated with certain health conditions. The following table shows the risk indicators that appear for hypertension, diabetes, and coronary artery disease (CAD) when the corresponding options are selected on the Risk Factors Config template.
