Add and Review Comments for the History of Present Illness
OpenNextGen® Enterprise EHR. Open theSOAPtemplate, and then expand theReason for Visitpanel. Select Comment. The HPI Notes template opens. Review and update the comments. Note: If you have copied and edited records from a previous encounter, a label appears on the HPI Notes template indicating that the corresponding record is copied and edited. The label appears only if you configure it in the Copy Previous Visit Information section of the Practice Configuration template. Select Save & Close. Edit a Reason for Visit on SOAP
After the intake staff documents the patient's reasons for visit, you can edit the reasons for visit on the SOAP template to document additional details. Open NextGen® Enterprise EHR. On the SOAP template, expand the Reason for Visit panel. To edit a reason for visit, do the following: Select the HPI that you want to edit in the Reason for Visit grid. Review Reasons for a Patient's Visit
You can review and update reasons for a patient's visit during the provider workflow. Open NextGen® Enterprise EHR. Open the SOAP, and then expand the Reason for Visit panel. Review and update the patient summary in the Introduction field. Review and update reasons for visit, diagnostic studies, and chronic conditions. To add any additional information, select Comments. Review Intake Comments on the SOAP Template Review comments entered during the intake if the Intake Comments link on the SOAP template indicates that content is present. Edit a Reason for Visit on SOAP You can edit existing reasons for visits on the SOAP template. Add and Review Comments for the History of Present Illness You can review and enter additional comments for a patient's history of present illness (HPI). Remove a Studies Reviewed on SOAP You can remove the historical diagnostic studies that are added to the history of patient illness in the Studies Reviewed section on the SOAP template. Document the Patient's Reason for Visit as a Provider
The reason for visit added by the intake staff on the Intake template appears on the SOAP template. The Introduction field is an editable field that displays a brief summary of the patient's reason for visit. Open NextGen® Enterprise EHR. On the SOAP template, in the Reason for Visit panel, do one of the following: Select one of the reasons for visit entered previously. Select a complaint from the reasons for visits in the left pane. The associated HPI template opens. Note: If you select reason(s) for visit using the Additional / Manage link, then the reasons appear in the Chief Complaint column in the data grid. You must select the reasons listed in the Chief Complaint column in the data grid to open the associated HPI template. Reason for Visit panel On the associated HPI template, enter the appropriate data. Enter any comments. Select Save & Close. You can review and modify the patient’s reasons for visit, review of systems, and vital signs documented by the intake staff on the SOAP template. You can also review the patient's visit type, alerts, and risk indicators. In addition, you can document the physical examination details, history, and plan information. You can review and modify the following details on the SOAP template. General information Reasons for visit History Review of systems Vital signs Physical examination Assessment and plan Although the you could enter the reason for visit and HPI on the Intake template, there are options available on the SOAP tab that are not available on the Intake tab. Manage Review of Systems on SOAP
You can add or edit the symptoms documented by the intake staff in the Review of Systems panel on the SOAP template. Open NextGen® Enterprise EHR. Open the SOAP template. To document multiple systems on one template, do the following: In the Review of Systems panel, select the one page ROS link in the left pane. Document the findings on the respective ROS template. Select Save & Close. To document the findings for a single system, do the following: In the Review of Systems panel, select a single system in the left pane. Document the findings on the system specific ROS template. Select Save & Close. To edit a system, do the following: In the Review of Systems panel, select a record.
