You can remove an entry from the grid in the
Documentation Log
panel.
Select an entry to be removed from the grid in the
Documentation Log
panel.
Click
Remove
.
An alert message appears.
Click
Yes
to confirm.
The selected row removes from the grid.

Add Comments on the ASC Documentation Log Entries
Comments can be added to entries documented on the ASC Documentation Log template. To add comments for an entry type, click Add/Edit Comments to open the Documentation Log Comments template. Select the desired row in the grid. Enter any additional comments in the Comments field and click Update. The comments appear under the Comments column in the grid. You can use the My Phrases link at the top-right of the Comments field to add comments in the Comments field. The My Phrases/ Common Phrases link allows you to create a phrase that displays in the list launched from the My Phrases link. Edit an Entry
You can edit the entries in the Documentation Log panel. Select an entry to be edited from the grid in the Documentation Log panel. Click Add. The ASC Documentation Log template opens. The row of entry selected in the Documentation Log panel appears in the left panes and top right grid of the template. Select the applicable value. If you want to clear a selected value, reselect that value. The asterisk (*) clears from the value. To make changes in the Time and/or Late Entry details in the grid: Select an entry in the grid. Make relevant changes in the Time field or Late Entry fields. Click Update. Click Save & Close. The modifications update in the grid in the Documentation Log panel. Documentation Log Panel on the ASC - Preprocedure Template
The Documentation Log panel is a centralized location to document patient's assessments, routine quick notes, medications administered to the patient, any adverse events that occurred during the procedure, and the nursing diagnosis and nursing assessments initiated for the patient. The Documentation Log panel is also available on the following templates: ASC - Intraprocedure ASC - Postprocedure ASC - Anesthesiology Documentation Log The Procedure field displays the first procedure documented in the current encounter. The Documentation Log panel includes the following filter fields: View by - Filters the entries by: All entries - Displays entries documented for the categories as a separate record. Remove an Entry Type from the Navigation Panel
In the Navigation panel, you can remove an entry type from the grid. On the Documentation Log Config practice template, expand the Navigation panel. Select a row in the grid. Click Remove. If the entry type has associated values, a confirmation message appears. To confirm, click Yes. The selected entry type removes from the grid. Document the IV Removal Details
Highlight a row of IV insertion record in the grid. Click Edit to open the ASC IV template. If the IV is removed, do the following in the Removal section: Click the drop-down next to the Removal time field. The Time Entry template opens. Note: The Removal time field is available to document only when the IV insertion attempt is Successful. Enter the time when the IV was removed and click OK. The documented time appears in the Removal time field. You can click the clock to enter the current system time. The Removed by field populates with the current user name. Click Removal site condition and select the condition(s) of the IV site after removing the IV. Click Close on the list. Click Reason for Removal and select a reason from the list. If appropriate, click Removed by and select the nurse or medical assistant who removed the IV from the patient. If applicable, enter or update comments in the Comments field. Update Documented Data in the Documentation Panel
In the Documentation panel, select a row in the grid. Click Edit to open the ASC Procedure Documentation template. Make the required changes by clicking any of the sections in the left pane, and then updating the selections in the grid. The selected values update to the field below the View label. Click Save & Close. The modified record updates in the Documentation panel grid. To delete a procedure documentation record, highlight a row in the Documentation panel grid and click Remove. If you want to save the documented values as the default template, click the Quick Save button and enter a default set name for the template.
