The Documentation Log panel is a centralized location to document patient's assessments, routine quick notes, medications administered to the patient, any adverse events that occurred during the procedure, and the nursing diagnosis and nursing assessments initiated for the patient.
The Documentation Log panel is also available on the following templates:
- ASC - Intraprocedure
- ASC - Postprocedure
- ASC - Anesthesiology
The Procedure field displays the first procedure documented in the current encounter.
The Documentation Log panel includes the following filter fields:
- View by - Filters the entries by:
- All entries - Displays entries documented for the categories as a separate record. This filter option sorts entries by time with the most recent entries at the top.
- All entry records - Displays a concatenated view of different categories documented in an entry. This filter option sorts entries by time with the most recent entries at the top.
- Flowsheet - Displays the vital signs documented in the Vital Signs Monitoring panel, and the quick assessments and/or medications records documented in the Documentation Log panel.
Note: To display the record in the grid, the entry time of the vital signs monitoring must match the entry time of the quick assessments and medications details.
- Group by Follow-up records - Displays follow-up records.
- Medications - Displays records documented under the Medications category.
- Entry Type - Displays entries associated with the selected Entry Type.
Note: To clear the filter, click the Entry Type filter field and select the top-blank row in the list.
- Category: Sub Category - Displays entries associated with the selected Category: Sub Category.

- Template - Displays entries documented on the selected template.
Note: To clear the filter, click the Template filter field and select the top-blank row in the list.
The Quick load entry time field is available to enter the time of the quick load before you load the default values using the quick load
button. After you click the quick load 33e list, the quick load time and the default values display in the grid.
The Medication Totals link opens the ASC Medication Totals template with total dosage of each medication documented in the current encounter.
