Update a Barrier to Care

Click to highlight the barrier to care on the Barriers to Care panel that you want to update. Click the Edit button. The Barriers to Care template appears populated with data for the selected barrier to care. Update the fields on the template as required. Click the Save & Close button. Record a Barrier to Care

Click the Add button for the Barriers to Care panel. The Barriers to Care template opens. Barriers To Care Click the Onset Date field and select the date the barrier to care began and click the OK button. The selected date is populated into the Onset Date field. Type a meaningful description of the barrier to care in the Barriers to care field. If the barrier to care has been resolved, click the Resolved Date field and select the resolution date from the resulting calendar. The Status field is pre-filled with a status of Active. To specify another status, click the field and select the new status from the resulting pick list. If you have resolved the issue that presented the barrier to care, you can select a status of Deleted. The barrier is marked as deleted but the information remains in the system for tracking purposes. Optionally, type additional comments in the Comments field. Barriers to Care Panel for the Care Coordination Home Page

The Barriers to Care panel enables you to record and track patient circumstances that may negatively impact the patient’s ability to comply with the requirements of the nursing care plan. Examples of barriers to care include: Lack of reliable transportation Difficulty in communicating Other medical conditions that inhibit movement or understanding Family issues that make care difficult To filter the list to show only records of a specific type, click the Filter field and select the filter criteria. You can filter records on the list by all, active, deleted, and resolved. If there are no barriers to care for a patient, you can select the None check box to record. When you select the check box, the system adds a record to the grid indicating that the patient has no barriers to care. If you subsequently add a barrier, the system marks the no barriers to care record as deleted. Edit a Barrier

Click Filter and select a status of the barriers from the Filter for Barriers to Care list. The barriers available on the selected status appear in the grid. Highlight a barrier in the grid and click Edit. The Barriers To Care template opens. Make relevant changes in the applicable fields. Click Save & Close. The modified details update in the grid. Remove a Referral

Click to highlight the referral on the Referral History panel that you want to remove. Click Remove. You are prompted to confirm the delete. Click Yes. You are returned to the Care Coordination Home Page with the selected referral removed from the Referral History grid. Parent topic: Referral History Panel

Remove an Agency or Team Member

You can remove an agency or team member at any time. When you remove an agency, any associated team member entry is also removed and when you remove a team member, any associated agency entry is removed also. Click to highlight the agency or member on the Care Coordination Team panel that you want to delete. Click Remove. You are prompted to confirm the delete. Click Yes. You are returned to your originating template with the selected barrier to care removed from the Agencies or Interdisciplinary team grid.
