Barriers to Care Panel for the Care Coordination Home Page
The Barriers to Care panel enables you to record and track patient circumstances that may negatively impact the patient’s ability to comply with the requirements of the nursing care plan. Examples of barriers to care include:
- Lack of reliable transportation
- Difficulty in communicating
- Other medical conditions that inhibit movement or understanding
- Family issues that make care difficult
To filter the list to show only records of a specific type, click the Filter field and select the filter criteria. You can filter records on the list by all, active, deleted, and resolved.
If there are no barriers to care for a patient, you can select the None check box to record. When you select the check box, the system adds a record to the grid indicating that the patient has no barriers to care. If you subsequently add a barrier, the system marks the no barriers to care record as deleted.
