Care Coordination Team Panel For the Care Coordination Home Page

The team members who are going to provide healthcare services to a patient must be defined on the Care Coordination Team panel. Care Coordination Team The Care Coordination Team panel contains two grids that display information about the defined team members. The top Agencies grid shows information about the agencies providing the care while the bottom Interdisciplinary Team shows information about individual providers who are involved with caring for the patient. In most cases, each team member will have an entry on both grids, one on the Agencies grid for the agency or practice the member is associated with and an entry on the Interdisciplinary Team grid for the actual team member. However, it is not uncommon to have agencies defined without having a specific provider identified or a provider who is not associated with a specific agency. Record a Barrier to Care

Click the Add button for the Barriers to Care panel. The Barriers to Care template opens. Barriers To Care Click the Onset Date field and select the date the barrier to care began and click the OK button. The selected date is populated into the Onset Date field. Type a meaningful description of the barrier to care in the Barriers to care field. If the barrier to care has been resolved, click the Resolved Date field and select the resolution date from the resulting calendar. The Status field is pre-filled with a status of Active. To specify another status, click the field and select the new status from the resulting pick list. If you have resolved the issue that presented the barrier to care, you can select a status of Deleted. The barrier is marked as deleted but the information remains in the system for tracking purposes. Optionally, type additional comments in the Comments field. Remove a Barrier to Care

Click to highlight the barrier to care on the Barriers to Care panel that you want to delete. Click the Remove button. You are prompted to confirm the delete. Click the Yes button. Update a Barrier to Care

Click to highlight the barrier to care on the Barriers to Care panel that you want to update. Click the Edit button. The Barriers to Care template appears populated with data for the selected barrier to care. Update the fields on the template as required. Click the Save & Close button. Barriers to Care Panel for the Plan/Intervention Tab

You can record and track patient circumstances that may negatively impact the patient’s ability to comply with the requirements of the nursing care plan using the Barriers to Care panel. The Barriers to Care panel on the Plan/Intervention tab functions exactly like the Barriers to Care panel on the Care Coordination Home Page. Barriers To Care Panel on the ASC - Pre admission Template

The Barriers to Care panel enables you to record and track patient circumstances that may negatively impact the patient’s ability to comply with the requirements of the care plan. Examples of barriers to care include such things as a lack of reliable transportation, difficulty communicating, the other medical conditions that inhibit movement or understanding, family issues that make care difficult, and so on. The Barriers to Care panel is shared with other templates and specialties. This information carries forward to care management or family practice templates. Barriers to Care The clinical support staff can review the documented data and make the applicable changes. If the case ID of the encounters is the same, then the details documented in this panel on the ASC - Pre admission template carry forward to Barriers To Care panel on the ASC templates of the next subsequent encounters with the same case ID.
