You can document about the team members who are going to provide healthcare services to a patient on the Care Coordination Team panel. Care Coordination Team The Care Coordination Team panel contains two grids that display information about the defined team members. The top Agencies grid shows information about the agencies providing the care while the bottom Interdisciplinary Team shows information about individual providers who are involved with caring for the patient. In most cases, each team member will have an entry on both grids, one on the Agencies grid for the agency or practice the member is associated with and an entry on the Interdisciplinary Team grid for the actual team member. However, it is not uncommon to have agencies defined without having a specific provider identified or a provider who is not associated with a specific agency. You can monitor the progress of nursing care plans by reviewing the interventions in the Progress panel. The panel lists all intervention encounters, but you can filter the list to show only those encounters specific to an identified problem, a defined goal and an intervention type. Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. All providers, staff, coordinators and so on involved in a patient’s care are indicated in this panel. On the Plan/Intervention tab, expand the Care Coordination panel. Select the required option. Enter comments in the Care Coordination panel. The Barriers to Care panel enables you to record and track patient circumstances that may negatively impact the patient’s ability to comply with the requirements of the nursing care plan. Examples of barriers to care include: Lack of reliable transportation Difficulty in communicating Other medical conditions that inhibit movement or understanding Family issues that make care difficult To filter the list to show only records of a specific type, click the Filter field and select the filter criteria. You can filter records on the list by all, active, deleted, and resolved. If there are no barriers to care for a patient, you can select the None check box to record. When you select the check box, the system adds a record to the grid indicating that the patient has no barriers to care. If you subsequently add a barrier, the system marks the no barriers to care record as deleted.
