Complete the Diagnoses Section for Transfer/Discharge

In the Behavioral Health Diagnoses section, select any of the available links: Diagnosis Code Lookup Add Common Assessment Mandated Care Coordination In the required Diagnostic description field, click to launch the diagnosis search window. Selecting a description value automatically populates the ICD Code field or vice versa. In the Status field, click to select a value from the list. If appropriate, enter a Severity. If needed, enter a Differential dx, and/or Impression. To enter Specifiers, click one of the following: In the field to manually enter any required values The My Phrases link. For more information about My Phrases, go to NextGen Healthcare Success Community, and download the Getting Started User Guide for NextGen® Adaptive Content Engine. For chronic conditions, select the option to Mark diagnosis as chronic. Select the options to add the assessment to the Clinical problems, My tracked problems, and/or My favorites where applicable. Enter Transfer/Discharge Data

The assessment and diagnosis information of the client being transferred or discharged can be entered on the BH - Transfer/Discharge 1 template. Select the Transfer or Discharge option. Enter a Date. Select from one of the available statuses or choose Other to type an explanation of the client's situation. If enabled, click the DSM IV link that appears in the upper, right corner to launch the BH – DSM IV template. The display of the link is setup on the Behavioral Health section of the Practice Configuration system template. Record a Discharge Diagnosis

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. Click the Care Transitions tab. The Cm Care Transitions template opens. Cm Care Transitions On the General panel, click a blank Discharge diagnosis field. The Diagnosis Search window opens. Use the window to locate and select the appropriate diagnosis. The selected record appears in the Discharge Diagnosis field in the Discharge Diagnosis panel. Note: You can add up to four diagnoses. Edit a Discharge Instruction in the Today's Discharge Instructions Grid

Highlight a row in the Today's Discharge Instructions grid. The selected discharge instruction appears in the Discharge Instructions field. Make relevant changes to the discharge instruction.
