Transfer or Discharge Visit Workflow

Set the Visit Type to Discharge Planning on the Home Page - Behavioral Health template to access the Transfer/Discharge tab, which includes three section tabs. The BH - Transfer/Discharge template set can also be accessed at any time by clicking Discharge/Transfer on the bottom section of the left navigation bar. Enter a Discharge Summary and Treatment

On the BH - Transfer/Discharge 2 (the Summary section tab in the Transfer / Discharge template set), describe the type of treatment received and how often in the Modality/frequency field. Type a specific Duration in the associated field. Describe the client's Compliance. Complete the Diagnoses Section for Transfer/Discharge

In the Behavioral Health Diagnoses section, select any of the available links: Diagnosis Code Lookup Add Common Assessment Mandated Care Coordination In the required Diagnostic description field, click to launch the diagnosis search window. Selecting a description value automatically populates the ICD Code field or vice versa. In the Status field, click to select a value from the list. If appropriate, enter a Severity. If needed, enter a Differential dx, and/or Impression. To enter Specifiers, click one of the following: In the field to manually enter any required values The My Phrases link. Enable the DSM IV Link

To document using the DSM IV standards, you must first set up the DSM IV link to appear on the Assessment/Diagnosis panel. The DSM IV link can be enabled on the Practice Configuration or the Documentation Configuration practice templates. You do not need to apply this setting on both templates. The settings applied on either template are shared and do not need to be repeated. Select the File menu and click System/Practice Template. Click the Practice tab and set Show to All. Select one of the following: Practice Configuration Documentation Configuration Scroll down to the Behavioral Health section. Under DSM IV/DSM-5 Assessment/Diagnosis Template Display Options, select the Show DSM IV link option. Click Save and Close at the top right. Enter DSM IV Assessment Details The BH - DSM IV template is used to document DSM IV assessments. Document the Patient's Transfer Summary

You can document the patient's transfer details from one program to another. Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. Select Transfer Summary (BH), and then select OK. Select the program, specialty, and visit type. Select the patient's discharge date. To close the selected program for the patient, select the status of the patient's treatment in the program. In the Assessment/Diagnosis panel, document the patient's active diagnosis and assessment details. In the Transfer Summary panel, document the treatment details, post-transfer support details, risk details, and the reason for the patient's transfer. In the Transfer Plan panel, document the patient's active medications and transfer plans and instructions. Select Generate Document.
