Documenting General Information of Patients

You can document a patient's general information on the Intake template. General panel on Intake When someone provides the patient details, you can document the historian details in theHistorianfield. When someone interprets the patient’s communication, you can select theInterpreter Detailslink to document the details of the interpreter. Intake Template

The intake staff uses the Intake template to document a patient's chief complaint, vital signs, medications, allergies, standing orders, and various other findings that may not have already been recorded on the Home Page - ORT template. Documenting General Information of Patients You can document a patient's general information on the Intake template. Document Interpreter Details You can document the answers for the pre-configured questions. Document Configurable Questionnaires You can document the answers for the preconfigured questions on the Additional Visit Information template. Patient's Reason for Visit You can document a patient's reasons for visit. Patient's History Summary You can add, update, delete, and review a patient's history summary details from the History Summary panel. Documenting Patients' General Information

You can document a patient's general information on the Intake template. General panel on Intake You can specify whether the patient is new or established for the current encounter in the General panel on the Intake template. If encounters exist for the patient, the Established patient option is automatically selected. When someone provides details on-behalf of patient, you can document the historian details in the Historian field. When someone interprets communication on-behalf of patient, you can select the Interpreter Details link to document the details of the interpreter. You can select the Configurable Questionnaires link to document the screening details of the patient. You can select the Transitioning into Care link to document the summary of care record details for a patient. You can select the Family planning encounter and Disability check boxes if an encounter is a family planning encounter and describes any disability, respectively. Patient's General Information

The General panel, on the Intake template displays if the encounter is for a new or established patient. The Established patient option is selected by default if the patient has previous encounters. You can document the person providing the patient information in the Historian field. When someone else interprets the patient’s communication, you can use the Interpreter Details link to document the details for Interpreter Details category. You can use the Configurable Questionnaires link to document the details for categories available in the Selected Category picklist. The Transitioning into Care link opens the Transitioning Into Care template that captures electronic Summary of Care requests and/or receipts that are completed electronically and document the information as reportable fields on the template. Document Interpreter Details

You can document the answers for the pre-configured questions. When someone interprets patient's communication, you can document the interpreter details on the Additional Visit Information template. Open NextGen® Enterprise EHR. Open the Intake template. Expand the General panel. Select the Interpreter Details link. The Additional Visit Information template opens. Additional Visit Information template Select a specialty. Note: By default, the Interpreter Details appears in the Selected Category. The configured questions for the Interpreter Details category appear in the Pre-configured questions for selected category(s) grid. You can select show all answered questions for this encounter check box to filter and view the questions that are answered. You can configure additional questions on the Additional Visit Information Config template. Document General Information

After reviewing the available patient details on the Home Page template, you can document the general information of a patient in the General panel. Open NextGen® Enterprise EHR. Open the Intake - Cardiology template. Click the General panel. Select the patient. If someone else is providing the patient details, select the relationship of the person with the patient in the Historian field. To capture the summary of care record information for a patient, click Transitioning into Care, and fill the information in the resulting window. For more information about Transitioning into Care, go to NextGen Healthcare Success Community, and download the latest Getting Started User Guide forNextGen®Adaptive Content Engine. To show the HCC links, badges, and grid columns on the templates, select Enable HCC for this patient. To indicate the disability, select Disability.
