Document Interventions, Methods, and Actions

Expand the Intervention(s)/Method(s)/Action(s) panel. In the Linked to goal field, click to select from the list of previously entered goals. Note: Linked to goal, Linked to Objective, and Intervention are required fields in this panel. In the Linked to objective field, select a previously selected objective. Enter the Intervention. In the Service description/modality field, select a type of therapy/group from the list or click on the top, blank line to type a value in the field. Set the Frequency of how often therapy and/or group sessions take place. In the Responsible field, select a type of provider from the Intervention Staff list or choose the top, blank line to free text a value. Select Active or Inactive. Click Add. Once the information has been added to the grid, additional interventions may be recorded as needed. When the IAP or BH Master document is generated, the intervention, method and/or action information appears under the status selections made on the BH - Individualize

Enter Methadone & Assertive Community Treatment Program Information

In the Methadone Treatment section, click Methadone programs only - attendance schedule to select a value from the list or select the blank row at the top of the list to type an entry in the field. Click the Daily dosage field to choose a value or select the top, blank row to type in the field. In the Assertive Community Treatment Programs section, enter information For Assertive Community Treatment programs only, indicate the changes in individual’s status in assessed domain. BH - Individualized Action Plan (IAP) Template

The Individualized Action Plan (IAP) template is the master treatment plan of care for the client. Following the Comprehensive Assessment exam, the Behavioral Health provider may choose to initiate an IAP. The IAP is completed in collaboration with the Behavioral Health client and a copy of the finished plan is usually signed by the client. The action plan template enables users to leverage electronic functionality by tying together the data across the client’s assessment, IAP, and Progress Note. Because this information is shared, the user is not repeatedly entering the same data. The BH - IAP Review/Revision Template

The BH - IAP Revision/Review template is used to document progress or lack thereof and demonstrates evidence and/or rationale for the amendment of treatment goals and objectives in the Individual Action Plan (IAP). The system automatically pulls forward data from the last IAP or IAP review to the current encounter. Opiate treatment programs are supported as part of Outpatient Mental Health & Substance Abuse Services. Several templates are available to help clinicians who treat patients suffering from opiate addictions.
