Start an IAP Revision/Review

To access the BH - IAP Revision/Review template do one of the following: Click the IAP Review tab. Click the IAP Review link in the navigation bar. Click the IAP Review link available from most of the Behavioral Health tabs in the template set. On the BH - IAP Revision/Review template, click the Program name field to select a value. Program names can be added and/or edited by the practice on the Custom Picklists Setup, system template. Set the Review date. Select Review or Revision. Enter a future date for Next review due by. The BH - IAP Review/Revision Template

The BH - IAP Revision/Review template is used to document progress or lack thereof and demonstrates evidence and/or rationale for the amendment of treatment goals and objectives in the Individual Action Plan (IAP). The system automatically pulls forward data from the last IAP or IAP review to the current encounter. Opiate Treatment Support

Opiate treatment programs are supported as part of Outpatient Mental Health & Substance Abuse Services. Several templates are available to help clinicians who treat patients suffering from opiate addictions. As a clinician, you can review and manage caseloads in the Opiate Treatment Clinician Management practice template. You can view a patient’s treatment history in the Opiate Treatment Program demographic template. Finally, complete documentation of treatment progress in the Opiate Treat Progress Note (BH) template. The default program option is a Methadone Treatment Program, but you can set up other treatment programs such as Suboxone. Document Treatment Recommendations/Assessed Needs

On the Treatment Recommendations/Assessed Needs panel, if appropriate, select No additional recommendations clinically indicated. To create a new entry, click the Priority field to enter a number. Type an Assessed Need. In the Status field, click to select a value from the Need status list. Enter a date in the Identified field. If applicable, enter Rationale(s) and/or a Resolved date. Click Add. Repeat steps 1 - 6 as needed. In the Further Evaluations Needed section, do one of the following: Select None indicated. Place a check mark next to any of the applicable options. Enter details in the Level of care/indicated service recommendations field. Type a summary in the Individual served/guardian/family response to recommendations field. In the Treatment Planning Updates section, select No or Yes to Change in IAP Required. If Yes is selected, complete the IAP revision by clicking IAP Review at the top of the template. Create Medication Taper Schedules

You can create, modify, and delete dosing tapers in the Medication Taper Schedules panel. Tapers are specific to the medication line. You must configure multiple tapers for medications with multiple doses. On the MAR Configuration (BH) template, expand the Medication Taper Schedules panel. Select a taper name. Note: The Taper Name picklist displays the list as configured in the BH Content panel on the BH Foundation Configuration template. Select a medication. The MEDID field displays the medication ID for the selected medication. In Day, enter the number of days to administer the medication. For example, if a medication is given twice a day for three days, a taper record for each day and time (day 1, day 2, day 3, time 1, and time 2) is created for a total of six taper entries. In the Med Time field, enter the time of day to administer the medication. Optional: To remove data from all of the fields, select Clear All. Select Add New.
