Document Health Promotion Plans

You can document the Health Promotion Plans such as BMI plan, depression plan, and hypertension plan of a patient. Open NextGen® Enterprise EHR. Open the Intake template. Expand the Vital Signs panel and then select the Health Promotion Plan link. The Health Promotion Plan template opens. Expand the Health Promotion Plan panel. Health Promotion Plan Add a diagnosis. Select Plan and select a plan from the list. Based on the selected plan, the different sets of fields appear. To add the plan details, do the following: For BMI Plan, add the diet, physical activity, and referrals details. For Depression Plan, dd the follow-up and referrals details. For Hypertension Plan, add the diet, physical activity, referrals, and lifestyle details. Note: To enable the Referrals section and order a referral, you must add a diagnosis. To indicate that the referral is internal, select the Internal referral check box. If required, in the Details field, enter any additional information. If you want to add

Set Up HPIs to Appear in the Reason for Visit

You can set up the History of Present Illness (HPI) to appear in the reason for visit. A practice can configure the Reason for Visit panel using the Framework Content system template. For more information on the reason for visit configuration, go to NextGen Healthcare Success Community, and download the latest Framework Content System Template forNextGen®Adaptive Content Engine. Open NextGen® Enterprise EHR. From the File menu, select System \ Practice Template. The Select Template window opens. In the Show section, select All. To open the Framework Content template, double-click Framework Content. Select Specialty and select a specialty from the list. Expand the Reason for Visit panel. In Filter, select the all option. Select the reason for visit linked to the appropriate History of Present Illness (HPI), which you want to add to the all list in the Reason for Visit list. Note: The reasons for visit are linked to an HPI template on the Manage Custom Links practice template. Select OK.

Document Pain Assessment and Plan of Care

You can document the pain assessment details and follow-up plan of care. Open NextGen® Enterprise EHR. Open the Intake template. Expand the Vital Signs panel, and then select the Health Promotion Plan link. The Health Promotion Plan template opens. Expand the Pain Management panel. Note: The Pain level and Method fields display the details entered on the Vital Signs template. You cannot update the pain level and method in the Pain Management panel. Pain Management - Health Promotion Plan To manage exceptions and view measure details on the Exceptions/Measures template, select Exceptions. If details are not entered in the Pain level section on the Vital Signs template, select the following fields, and select the appropriate option in the list. Pain level Method Duration Quality To indicate to see the HPI details, select the See HPI check box. Enter the location of the pain of the patient. Select Onset, and select the date when the pain started. Enter the follow-up plan of care for a pat

Set Up Template Preferences

You can configure the default and preferred templates to automatically open the templates for an encounter. You can also configure the workflow after opening an encounter from the Workflow Appointment list. Open NextGen® Enterprise EHR. Select Tools > Preferences. The Preferences window opens. Preferences window Select the Templates tab. To move a commonly used template in the preferred list, select the template in the Available Templates list and then select the right arrow. The selected template appears in the Preferred Templates list. To select all templates, select the double-right arrow. To set a default template, from the Default Template list, select a template. Note: NextGen Healthcare recommends setting the Home Page (fts_home_page) template as a default template which provides a clinical summary of the patient to the provider. Create Custom Health Promotion Plans

You can create a custom health promotion plan. Open NextGen® Enterprise EHR. Open the Template Data Sets Setup template. In Plan Type, enter a label for the kind of plan you want. In File Maint. Dx Category, enter a disease category set up in File Maintenance. In Label for Value, enter a label that you want to search. Select the data tables and data fields that you want to retrieve values from. In Lo Value and Hi Value, enter low and high values. In Units Value, enter the type of units to display. In Other DBPicklist Name, select up to 10 custom lists. Note: The Developer, Funding Source, and Reference Updated fields support clinical decisions. You can enter the developer and funding source names in the Developer and Funding Source fields respectively. When you add the configuration details in the other fields, Reference Updated automatically fills with the current date. Select Save. Create Customized Health Promotion Plan

On the Data Sets Setup template, click Data Set and select Health Promotion Plan. The existing data set items associated with the Health Promotion Plan data set appear in the grid. Health Promotion Plan data set Click Add. The Template Data Sets Setup – [New Record] template opens. Template Data Sets Setup If applicable, click Diagnosis description to search and select a diagnosis. The associated diagnosis code automatically populates. If applicable, click Provider and select a provider. Enter the details in the necessary fields. Click Save, and then click Close. The newly configured data set item appears in the Data Sets Setup template. Note: To add another item, click Clear For Add in the Template Data Sets Setup – [New Record] template and repeat steps 1 and 2.
