SNOMED Codes

Quality Reporting Stage 2 regulations require a patient’s chronic problems to be coded using SNOMED . The Systematized Nomenclature of Medicine - Clinical Terms ( SNOMED CT®) is a comprehensive collection of clinical terminology that was originally created by the College of American Pathologists. It contains healthcare concepts with unique meanings and formal, logic-based definitions organized into hierarchies. The SNOMED Codes provides the ability to consistently index, store and retrieve clinical data across specialties and sites of care. The SNOMED information is mapped to ICD codes. Access the SNOMED Config Practice Template

From the File menu, select System \ Practice Template. On the Select Template window, click the Practice tab. In the Show section, select All to see a complete list of practice templates. To access the SNOMED Configuration practice template: Double-click SNOMED Configuration. Select SNOMED Configuration and then click OK. The SNOMED Configuration practice template appears. Create and Manage SNOMED Codes

You can create, add, modify, and manage SNOMED® codes in the SNOMED Configuration practice template. The data configured on the SNOMED Configuration practice template appear throughout NextGen® Adaptive Content Engine. Add a New SNOMED Code Mapping

You can add a SNOMED mapping in the SNOMED Configuration practice template. Access the SNOMED Configuration practice template. Right-click the SNOMED grid and select Add New. The SNOMED Mapping - [New Record] template opens. Enter a field name, and a symptom in the appropriate fields. Enter a concept in the Concept ID 1, Concept ID 2, and Concept ID 3 fields. Enter a SNOMED code in the SNOMED Code 1 and SNOMED Code 2 fields. Click Saveand click Close. The information entered appears in the SNOMED grid. Sort the SNOMED Mappings

You can sort the list of SNOMED mappings in the SNOMED Configuration practice template by column, including symptom, name of the field mapped to, any of the concept IDs, or SNOMED Code. Access the SNOMED Configuration practice template. Right-click on the SNOMED grid and select Sort Special to sort the information by column, in ascending or descending order. The Sort Special window appears. Click a column name in the Sort Special window. Select the Asc. or Desc. option. Select the applicable check boxes. Click OK. Modify a SNOMED Code Mapping

You can modify a SNOMED mapping in the SNOMED Configuration practice template. Access the SNOMED Configuration practice template. Select an entry in the SNOMED grid that you want to modify. Right-click the entry and select Open to edit the information. Note: Click Delete to delete the entry. Make the required changes. Click Saveand click Close.
