Enter the Vital Signs Monitoring Details

  1. Open NextGen® Enterprise EHR.
  2. Open a patient's record, and then open an encounter.
  3. On Top Toolbar, select Templates .
    The Select Template window opens.
  4. Open the ASC-Intake template.
  5. In the Vital Signs Monitoring panel, click Add.
  6. To indicate the baseline set of vitals, click the Baseline check box. .
  7. To enter the time when the vitals were taken, in Time, do one of the following:
    • Click the Time field arrow and enter the time on the Time Entry template.
    • Click to enter the current system time in the Time field.
  8. Enter the measured vitals in the following fields: Temp within the 50 – 121.98 F range, Pulse within the 10 – 499 range, Resp within the 0 – 99 range, Sys BP and Dias BP within the 0 – 399 range, SpO2 within 0 – 100 range, CO2, ECG rate, and ECG rhythm.
  9. To enter the height and weight of the patient, select the Height and Weight check boxes, and then enter the height of the patient within the 30.48 – 304.5 cm, 1 – 9.99 feet, or 12 – 119.88 in range and enter the weight of the patient within the 2.00 – 699.99 kg or 4.409 – 1543.3 lbs range.
  10. Click Pain level, and then select the intensity of pain from the list.
  11. Click Level of consciousness, and then select the patient's neurological status from the list.
  12. Click Bleeding severity, and then select the severity from the list.
    If the options in the list are not relevant, click the blank row on of the list, and then enter the severity in the Bleeding severity field.
  13. Click the Blood loss field arrow , enter the amount (in ml) of blood loss.
  14. To display the name of the user monitoring the patient's vitals during the procedure, select the Monitored by check box.
    The Monitored by field displays with the name of the user currently logged on to the system. You can click the Monitored by field and select the applicable user name from the list.
