- In the Plan and Additional Information panel, click My Plan to open the My Plan template set.

Goals and/or objectives that were entered previously on the Comprehensive Assessment templates populate in this panel. The grid on this panel offers a quick view of the client's current goals and objectives. In the Goals/Objectives Addressed As Per Individualized Action Plan section, Goal today field, click to select a previously entered item. Select the Objective today from the list previously entered objectives. Click Add. In the Interventions Addressed Today section, click Select From Previous IAP Intervention to choose a previous entered value. Enter the Interventions/methods provided. Record the Response to Interventions/progress toward goals and objectives. Document the Issues referred to physician/psychiatrist for consideration. On the Plan and Additional Information panel, do one of the following actions: Click the Plan/additional information field to manually type relevant information. Use the My Phrases functionality to enter a sentence or part of a sentence in the field. Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. You can view the list of actions performed during the visit in the Action Items panel. Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. The interventions appear only if they are added in the Nursing Care Plan Data template in the History panel. On the Plan/Intervention tab, expand the Interventions panel. Select an appropriate entry from the Interdisciplinary Team Member column. The system displays the interventions designated for the selected team member type. Interventions Select the intervention that you want to update and click Add. The Nursing Care Plan Data template opens. The template is populated with the data for the selected intervention. Nursing Care Plan Data Note: The system populates the Team Member field with the name of the current user and the Intervention frequency field with the currently defined intervention frequency. Use this template to add as many impressions, comments, differential diagnoses, and patient and provider’s plans for a diagnosis, as necessary.
