Document Admission Data

On the BH - Screening/Admission Note template, expand the Program Name panel. Click the Program name field to launch a list of choices. This list is not required and is customizable on the Custom Picklists Setup, system template. Set the Admission date. Enter a Service plan due date that is in the future. Click the clock button to enter the current time in the Start field. The Start and End buttons are available here and in the Behavioral health billing section at the bottom of the Screening/Admit tab. The values documented in either place are immediately reflected in the same fields on the other section. The entered times can be modified by clicking in the field. If desired, you can document a time at a later date. It is important to follow the established format: 0:00 AM or PM. Parent topic: Screening and Admission Visit Workflow

Add Additional Notes

On the Notes panel, enter a Narrative. In the Results of the Psychiatric Rehabilitation Readiness Determination field, click to enter a score and any other pertinent information. Enter details in the Strengths field. In the Clinical, immediate and other service related needs field, click to document the client's needs based on the referral information provided. Use the Rehabilitation aspirations field to relate the client's goals and desired outcome. Select No or Yes for Admission indicated and if Yes, enter a reason and/or disposition. Referral information can also be entered in the field. To order a referral for the client, click the Referral button to open the My Plan tabbed template on the Referrals tab. Use the Initial services field to free-text the services and/or activities that are provided to the client to meet the individual’s identified needs. Screening and Admission Visit Workflow

The BH - Screening/Admission Note template is used to document information that a program or Emergency Room might need for their initial evaluation of a client. Information gathered here is the basis for a summarization of the services and possible interventions that are initially provided to the client. Generally, this is completed by the non-clinical staff at the practice and can be done separately from a Comprehensive Assessment exam. The BH - Screening/Admission Note template can be accessed at any time during an encounter by selecting Other Templates from the bottom section of the navigation bar. Generate a Referral Document

Click to highlight the referral on the Referral History grid for which you need the document. Click Referral. The system generates and displays the requested referral document. Parent topic: Referral History Panel

Document Individuals Present

In the Individuals Present panel, select from one of the Contact type values. In the Individuals Present section, do on of the following: Select Individual not present and a Reason. Select Individual present. Select Others present and document their names. Enter the client's closest friends and relatives in the Support Resources section and click Add. Note: The First name and the Effective date are required To set the filter, click Active to view just the current resources or click All to see every resource ever associated with this client.
