Document Findings of Holter Monitors

The Intake staff documents the findings of the holter monitor test in the Findings section. Open NextGen® Enterprise EHR. Open the Holter Monitor template. Click the Procedure Information panel. In the Findings section, enter the Minimum, Maximum, and Average values for Heart Rate. Enter the values for Pauses, Supraventricular Events, and Ventricular Events. Document Holter Procedures

You can record the Holter Monitor test details. The Intake staff documents the procedure information which includes pre-procedure care, test/patient data, vital signs, stress data, recovery data, and wall motion. Open NextGen® Enterprise EHR. Click the Intake tab. Select Cardiology as Specialty and Testing Only as Visit Type. Click the Holter link. Note: You can also access Holter Monitor template from the left Navigation bar. The Holter Monitor template opens. Holter Monitor-Procedure Information Click the Procedure Information panel. Click the Indication field and select an indication. Click the Hook Up Date field and select the date when the holter monitor has been hooked. Click the Disconnect Date field and select the date when the holter monitor has been disconnected. Click the Send Copy To field and select a physician to whom you want to send a copy of the test report. Click Ordering Dr and select the ordering physician. Click Diary and select an option. View Midmark Holter Reports

You can view Midmark Holter, full disclosure, and summary reports and edit full disclosure reports. Reports are displayed in the Report Review window. Note: Some Midmark Holter functions require the use of a security key. For more information on how to use Midmark Holter functionalities, refer to the appropriate Midmark operation manual. Open the Integrated Equipment Module From the IEM grid, select View Report in a row that contains a Midmark Holter report. The Holter Monitor Options window opens. From the Holter Monitor Options window, you can: View and edit the Full Disclosure report (requires a security key). View the Summary report (no security key required). Select Re-Analyze to repeat the Holter scan on the original ECG recording (requires a security key and does not require the original CompactFlash card). Select Delete Full Disclosure Report to delete the Full Disclosure report and convert it to a Summary report (does not require a security key). Select Exit to close the Holte

Create Midmark Holter Reports

To acquire Midmark Holter data, your workstation must have a security key installed and the ability to read a CompactFlash or SD card. For more information on how to use Midmark Holter functionalities, refer to the appropriate Midmark operation manual. Prepare the patient for the test as needed. Insert the CompactFlash or SD card with the recorded Holter data into the card reader. In NextGen® Enterprise EHR, load a patient and select a location and provider. Open the Integrated Equipment Module From the Acquire Report list, select Midmark Holter. The Holter Analysis window opens. On the Holter Analysis window: Enter the recording Start Time & Date. This time is in 24-hour format (for 1:00 pm., enter 13:00). Use the right arrow key to move from the hour digits to minute digits. To generate reports, check the Print Reports After Analysis box and then select Report Settings to choose which reports to print. Select the Flash Card option as the data source and select Browse. Document Summary of Echocardiogram Tests

You can document a summary of the Echocardiogram test in the Summary panel. Open NextGen® Enterprise EHR. Click the Intake tab. Select Cardiology as Specialty and Testing Only as Visit Type. Click the Echo link to access the Echocardiogram template. In the Summary panel, click All Normal if all the results are normal. The Summary fields display the default statements for normal results. Enter a brief summary in the Summary field. Select an appropriate option from the list. Click the S icon to frame a sentence using the Sentence Builder template. Parent topic: Echo

Document Echocardiogram Procedures

You can document the Echocardiogram procedure from the Echocardiogram template on the Echo navigational link. Open NextGen® Enterprise EHR. Open the Intake template. Select Cardiology as Specialty and Testing Only as Visit Type. Click the Echo link to access the Echocardiogram template. Note: You can also access the Echocardiogram template using the Echo link in the Navigation bar. Procedure Information Select the appropriate check box in the Pre-Procedure Care section. Enter the required information in the Test/Patient Data section. Document the vital signs of the patient. Note: The vital signs information automatically displays if it is already documented in the Home Page or Intake template. Enter appropriate values in the following columns. Technical Details Aortic Valve Mitral Valve Tricuspid Valve Dimensions Pulmonic Valve Additional Findings In the LV Systolic Function section, click each text box on the diagrams and select a value from the list.
