Document Adverse Childhood Experiences Screening

You can document Adverse Childhood Experiences (ACE) of patients aged between 0 to 19 years using the Adverse Childhood Experiences (ACE-Q) template.

  1. Open NextGen® Enterprise EHR.
  2. For a patient encounter, open a template.
    For example, Intake
  3. In the navigation bar, select Screening Tools.
    The Screening Tools template opens.
  4. Select Adverse Childhood Experiences (ACE-Q).
    The Adverse Childhood Experiences (ACE-Q) template opens. The encounter date appears as the Performed Date by defaults.
  5. To enter a different Performed By date, select the field, and then select a date.
  6. To indicate that the questions are reported by the patient, select the Reported By Patient check box.
    The questions for your child are updated for you in the Additional Early Life Stressors panel.

  7. In the Original Ten ACE Questionnaire panel, answer all the questions.
  8. In the Additional Early Life Stressors panel, answer all the questions.
  9. In the Relevant Symptomatology panel, select any check boxes of relevant symptoms.
  10. In the Interpretation of Results section, select Calculate Score.
    A two-digit score appears in Total Score. Based on the combined Total Score field, a default interpretation appears in Interpretation.

  11. To override the calculated interpretation, select a new Interpretation.
    A note appears indicating that the interpretation is overridden.

  12. To enter a referral, select the Referrals link.
  13. To send a task, select Quick Task or Send Task.
  14. Select Save & Close.
