You can document the confidential sexual history of patients and their partners on the Confidential - Sexual History template.
The documented information on this template is automatically added to the Patient Information and Partner Information grids on the Details panel.
- Open NextGen® Enterprise EHR.
- To open the confidential templates, do one of the following:
- In the Social panel on the Histories template, select the Confidential History link.
- In the History Summary panel on the Intake template, select the Confidential link.
The Confidential - Alcohol/Drug template opens displaying the links to the confidential templates, in the left pane.
- Select Sexual Practices/STIs and then select the Detailed Sexual History tab.
The
Confidential - Sexual History template opens.
Note: The
Detailed Sexual History tab is available for documentation only if the
Enable Detailed Sexual History option is selected on the
Practice Configuration template. For more information, go to

- To include the confidential information in the master documents, select the Include all confidential information in the social history document with patient permission check box.
- Expand the Sexual History panel and document the patient's and their partner's confidential sexual history. For the patient's history, you can select Yes or No. For their partner, you can select Yes, No, or Unknown.
- Select OK.
