Add Additional Detail to an Entry

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. On the Functional Status Summary tab, expand the Functional Status Summary panel. Right click the corresponding entry and click Open. The Care Management Note template opens. Type additional notes in the Details text box. You can view a summary of all current care management orders in place for the patient. The template consists of a single panel, the Functional Status Summary panel, that displays a summary of the five categories being tracked for the patient. Create a DME Order

Outside DME (Durable Medical Equipment) can be documented on the Pt Dme template. The Pt DME template can also be launched from the Outside DME Order link in the navigation bar. On the Objective tab, navigate to the Documents and Printing panel. Click the DME button to open the Pt DME template. Pt Dme Highlight an assessment in the grid located in the Encounter Diagnoses section to associate with the order. Note: An alert message appears if you attempt to enter a DME type without previously selecting an assessment. Select medical equipment by category of Assistive devices, Supplies, and Splints. Click the Assistive Devices field and select from the picklist. The service item CPT code and ICD code appear in their respective fields automatically. If appropriate, click the Side field and select from the picklist. Repeat step 4 until all items have been selected. Document the Prognosis by selecting one of the available options. Document Transitioning into Care from Any Specialty

You can document transitioning into care from any specialty impacted by transition into Care Management. Open NextGen® Enterprise EHR. Select a patient and open an encounter. Open the Intake template. Select a Specialty that is impacted by Transitioning into Care Management and select Visit Type as Transition into Care. In the General Information panel, click the Transitioning Into Care link. The Transitioning Into Care template opens. Transitioning Into Care On the Transition into Care Management panel, click the Discharged field, and select the discharged date from the calendar. Click the Initial Communication field, and select the appropriate option from the list of communication types. Click the on field, and select the date of the initial communication from the calendar. Document Details of External Equipment

You can document the order details for durable medical equipment (DME) that must be purchased from external sources for orthopedic patients. Open the Home or any other applicable template, select Orders from the upper navigation bar, and then select Single Order. Expand the External DME panel. Select the item to order from the Item grid. The selected item appears in the Item field. Note: You can add, update, and delete items in the Item grid. By default, the current date appears in Date. Select the duration of the item, the date of the injury, and the date of surgery. Enter the reason for the order. Explanation of the Functional Section Tab

The information documented on the Functional section tab helps in determining the client’s needs and setting up goals and objectives that are an integral part of the recovery plan. Each panel on the Functional section tab is structured similarly. Items are listed and the question is asked of each item: Not clinically indicated (NI) and Current need area (CN). To understand the difference between NI and CN, look at this example of each in case of Individual care skills: NI - The client shaves and maintains a clean appearance. CN - The client does not want to wear any clothes. A field is available to right of each item in which evidence to justify whether NI or CN can be typed.
