Create a New Functional Status Record

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. On the Functional Status Summary tab, expand the Functional Status Summary panel, and then click Add/Update. The Cm Functional Status Input template opens. To record current therapies, open the Current therapies (home and outpatient) panel and select or enter the appropriate data for the patient's current therapies. Use the Document Panel to Complete a Visit

Before you begin Ensure that you have selected the Specialty as Care Management and Visit Type as Case & Consent. On the Documentation template, expand the Document panel. Document Add or update the diagnosis code as needed. Choose the appropriate type of visit for the encounter. Click the Add button. Generate the visit note by clicking the correct document link. Add a Document Entry

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. On the Plan/Intervention tab, expand the Document panel. Document Click the Diagnosis field and use the resulting search screen to select the appropriate diagnosis. Click the appropriate Type of visit check box. If you provided a nursing care plan to the patient or family, select the Nursing care plan given to patient/family/care giver check box. Click Add. The system generates an entry on the data grid. Generate Care Management Documents

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. You can generate the following documents from the Document panel: Nursing Care Plan Summary Transition of Care Comprehensive Assessment Return Visit Care Management Summary On the Plan/Intervention tab, expand the Document panel. To add additional records, fill in the fields as appropriate and click Add. To generate a nursing care plan Summary document, click the Generate Nursing Care Plan Summary link. To generate a transition of care document, click the Generate Transition of Care link. To generate a comprehensive assessment document, click the Generate Comp Assess link. To generate a return visit document, click the Generate Return Visit link. To generate a care management summary document, click Care Management Summary.
