Counseling and Coordination of Care for E&M Coding
Under the 95 or 97 Coding Guidelines, when the majority (50 percent or greater) of the encounter is spent in counseling and/or coordination of care with the patient and/or family, time should be used as the key or controlling factor to qualify for a particular level of E&M service. The application overrides the E&M level calculations based on history, exam, and Medical Decision Making (MDM) and calculates the E&M code based on the total time entered.
