Calculate E&M Codes under 2021 Coding Guidelines

You can calculate the Evaluation and Management (E&M) code under 2021 Coding Guidelines in Evaluation and Management.

You can base the Evaluation and Management (E&M) code calculation on the selected Medical Decision Making (MDM) level or total visit time.
  1. Open NextGen® Enterprise EHR.
  2. For a patient record, open a new encounter.
  3. Open the Finalize template.
  4. On the Evaluation and Management Coding panel, to calculate the E&M code based on the MDM level, review the prepopulated complexity level or manually select the level of complexity associated with the encounter in the Medical Decision Making section.
    The Medical Decision Making section displays the MDM level based on the calculated levels in the three tables in the Medical Decision - Making Elements panel. To change the MDM level, change the automatically-calculated MDM levels in the MDM tables, or by making corrections in the encounter chart note.

    For guidance on using the 2021 Coding Guidelines based on MDM level and time, select the information icon.

    For guidance on the level of complexity and risks involved in MDM, select the View MDM Guidelines link.

    The corresponding code is selected in the code options section at the bottom of the Evaluation and Management Coding panel. The code appears in the first Visit code field and in the Calculated E&M field.

    If counseling time is documented on the Counseling Details template, the Master or Specialty Masterdocument includes a statement regarding the counseling time spent in minutes.

  5. To calculate the E&M code based on the total visit time, do the following:
    1. Enter the counseling and educational details and factors.
      The total time spent for counseling appears in the Total counsel time (minutes) field.
    2. Select the Time (min) field, enter the total time in minutes including counseling time, and then select OK.
    The minimum time is 15 minutes for a new patient and 10 minutes for an existing patient.
    The application selects the corresponding code in the code options section at the bottom of the Evaluation and Management Coding panel. The code appears in the first Visit code field and in the Calculated E&M field.

    The Master or Specialty Master document includes a statement regarding the counseling time spent either face-to-face or non-face-to-face in minutes, by the physician or qualified health care professional.

    If counseling time is documented on the Counseling Details template, a message appears stating the counseling time spent in minutes.
