Components of the Physical Exam Panel

Button/Link	Description
One page Exam	Opens the gender-specific One Page- Physical Exam template to document a complete exam on one template.
Select Other Physical Exams	Opens a list of single-system exams to document a single system in detail.
Image	Opens the GYN image list and displays an image of the selected option. The provider can edit the image by adding text and color. Note: On the Physical Exam panel, the Image button appears only for female patients.

Document Multi System Exam on SOAP

You can document the complete physical examination details of a patient on the SOAP template. The One Page Exam link in the left pane on the Physical Exam panel enables you to open the One Page - Physical Exam template. Note: Based on how the One Page Exam template is configured in the Physical Exam section on the Framework Content system template, and whether the Enable Gender Neutral PE check box is enabled in the SOGI/Transgender section on the Practice Configuration template, a gender-specific or a gender-neutral One Page - Physical Exam template opens. Documenting or Annotating the Image

While completing the physical examination of a patient, you can annotate the exam details on the respective image of the system in the Physical Exam panel on the SOAP template. The Image button in the Physical Exam panel enables you to view a list of the available images, and when you select an image from the list, it displays the image. Copy Physical Exam Findings from Previous Encounters

You can copy physical exam findings from previous encounters of a patient to the current encounter instead of manually adding them. OpenNextGen® Enterprise EHR. Open theSOAPtemplate, and then expand thePhysical Exampanel. Select Physical Exam History. The Historical Information window opens. Note: The records are displayed for the default duration configured in the Copy Previous Visit Information section of the Practice Configuration template. To filter the records in the grid, enter the search criteria inFilter by, and then selectSearch. Document Pap Physical Exam Details for Annual Well Women Visits

You can document the physical exam details of an annual-well-women visit with a Pap specimen obtained and sent to the lab. Open NextGen® Enterprise EHR. Open the SOAP template. In the Physical Exam panel, select One Page Exam. The One Page - Physical Exam template opens. Select the GYN Exam link. The GYN - PE template opens. GYN - PE To indicate normal findings for each system, select the Normal check box. To add any abnormal information, select the corresponding Findings field. In the STI Screenings section, select Yes for Pap. To document the Pap specimen collection, Select the Detail link. The Pap Detail-PE template opens. Pap Detail-PE Enter the necessary details in Pap Detail PE, and then select the diagnosis and type of Pap. Select Save & Close. The Pap Detail-PE template closes. The Submit breast exam to Superbill check box is selected by default. Note: If a screening exam is performed, the system automatically submits the appropriate assessment and G-code when the Submit breast

Document a Breast Palpation Physical Exam

You can document the breast palpation details in the Physical Exam section on the SOAP template. Open NextGen® Enterprise EHR. Open the SOAP template. In Physical Exam, select One Page Exam. The GYN-PE template opens. GYN-PE template In the Breast section, under Palpation, select the Details link. The Breast Palpation-PE template opens. Breast Palpation-PE template Enter the breast palpation details in the corresponding breast, that is, right, left, or bilateral. Enter the breast palpation and location details. Physical Exam Panel

The Physical Exam panel displays a list of exam categories in the left pane. These items are customizable by specialty or by provider. The One Page Exam link opens the Derm General - PE template where you can document the patient's physical exam. You can also access the system-specific physical exam templates by accessing the links in the left pane. If the required system is not available in the left pane, you click the Additional link and select a system from the picklist. Click the Office Diagnostic button below the Physical Exam grid to launch the Office Services template to place orders for the tests. When you click the Physical Exam History button, the Historical Information window opens. You can copy the previous physical exam records to the current encounter by using the Copy Physical Exam & Close button.
