, enter the code type. For example, LOINC to indicate that the code is LOINC code.

Procedure Code
, enter CPT or NextGen Compendium Test keys.

, enter the description of the procedure code.

The newly configured data set item displays in the

template.

To add another item, click

Remove LOINC Codes Data Set Items

Add PT Modifier 59 Data Set Items
On the Data Sets Setup template, click Data Set and select PT Modifier 59. The existing data set items associated with the PT Modifier 59 data set appear in the grid. PT Modifier 59 data set Click Add. The Template Data Sets Setup – [New Record] template opens. Template Data Sets Setup If applicable, click Diagnosis description to search and select a diagnosis. The associated diagnosis code automatically populates. If applicable, click Provider and select a provider from Dbp Rendering Provider. In Code1, enter the CPT code for the first procedure. In Code2, enter the CPT code for the second procedure. In Modifier, enter the modifier as 59. Note: To bill the secondary service or the procedure that is not a part of the primary service or procedure, the modifier 59 is added. In Code Match 1, enter the primary code with which the modifier 59 is attached and sent for billing. In Code Match 2, enter the secondary code with which the modifier 59 is attached and sent for billing. Click Save, a

Add PE Copy Forward Excluded Data Set Items
On the Data Sets Setup template, click Data Set and select PE Copy Forward Excluded. The existing data set items associated with the PE Copy Forward Excluded data set appear in the grid. PE Copy Forward Excluded data set Click Add. The Template Data Sets Setup – [New Record] template opens. Template Data Sets Setup If applicable, click Diagnosis description to search and select a diagnosis. The associated diagnosis code automatically appears. If applicable, click Provider and select a provider from the resulting list. In Specialty, enter the specialty. In Template Name, enter the name of the template used for physical examination. In Field Name, enter the name of the field that is required to be excluded when you copy forward details. Click Save, and then click Close. The newly configured data set item appears in the Data Sets Setup template. Map SNOMED Code with the Diabetic Eye Exam Procedure
On the Data Sets Setup template, click Data Set and select Quality Reporting Order Findings xref. The existing data set items associated with the Quality Reporting Order Findings xref data set appear in the grid. Data Sets Setup Click Add. The Template Data Sets Setup – [New Record] template opens. Template Data Sets Setup If applicable, click Diagnosis description to search and select a diagnosis. The associated diagnosis code automatically populates. If applicable, click Provider and select a provider from Dbp Rendering Provider. In xref type, enter the system reference table name. In procedure code, enter the procedure code. In finding value, enter the physical examination value. For example, Negative, Normal, and so on. In code value, enter the SNOMED code. Edit LOINC Codes Data Set Items
On the Data Sets Setup template, click Data Set and select LOINC Codes. The existing data set items associated with the LOINC Codes data set appear in the grid. Highlight a grid to modify. Add Test Code Data Set Items
On the Data Sets Setup template, click Data Set and select Test Codes. The existing data set items associated with the Test Codes data set appear in the grid. Test Codes data set Click Add. The Template Data Sets Setup – [New Record] template opens. Template Data Sets Setup Click Test and select a name that the code or description will be mapped to from the picklist. The picklist displays the list of all test names currently available in the system. The name is used by the practice to identify an order, patient procedure, or lab. Click OK. Note: To enter a test name that is not listed, select the blank line that displays in the picklist and click OK. Type the name in the Test field. Enter one of the following: Codes - Enter the code to be mapped to. The entered code is used by the reference lab or outside study/procedure location to identify the test at their location for ordering or billing purposes.
