Chronic Conditions

You can view the chronic condition details of a patient from the Reason for Visit panel. The Chronic Conditions template enables you to document the chronic conditions for the patient. In addition, you can view and associate the patient’s active medications with three active diagnoses (that is, Diagnosis 1, Diagnosis 2, or Diagnosis 3) using the Associate Meds link available on the Chronic Conditions template. Document Chronic Conditions You can document patient's chronic condition details in the Reason for Visit panel. Associate Medications The Associate Medications template enables you to view and associate an active medication with up to three active diagnoses. Review Reasons for a Patient's Visit

You can review and update reasons for a patient's visit during the provider workflow. Open NextGen® Enterprise EHR. Open the SOAP, and then expand the Reason for Visit panel. Review and update the patient summary in the Introduction field. Review and update reasons for visit, diagnostic studies, and chronic conditions. To add any additional information, select Comments. Review Intake Comments on the SOAP Template Review comments entered during the intake if the Intake Comments link on the SOAP template indicates that content is present. Edit a Reason for Visit on SOAP You can edit existing reasons for visits on the SOAP template. Add and Review Comments for the History of Present Illness You can review and enter additional comments for a patient's history of present illness (HPI). Remove a Studies Reviewed on SOAP You can remove the historical diagnostic studies that are added to the history of patient illness in the Studies Reviewed section on the SOAP template. Document Alcohol and Caffeine Use

You can document alcohol and caffeine consumption for the patient. Select Alcohol/Caffeine on the side navigation. Enter information about the patient’s alcohol and caffeine use in the applicable fields. Social History template with Alcohol/Caffeine documented For the question Do you drink alcohol?, if you select the Yes or Formerly option, additional fields open so you can enter information on type and amount of alcohol. Enter Toxicology Screen Data

On the BH Substance Use Assessment template, expand the Toxicology Screen panel. Click Yes or No for Toxicology screen completed. To enter detailed results, click the Results, if available field. Document the Patient's Mental Health and Risk Assessment Information

Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. From the list, select Comp Assess - BH Hx, and then select OK. To document the patient's medical and surgical details in the Past Medical/Surgical panel, select Add.
