You can select a family health history from a group of health conditions.
- Open NextGen® Enterprise EHR.
- Do one of the following to open the Family Health History template and document the family history:
- On the Histories template, expand the Family panel, and select Add.
- In the History Summary panel on the Intake template, select Family, and select Add.
- Select a grouped condition with a plus (+) sign next to the condition.
The
Details 
icon appears next to the condition.
- Select
and select a secondary diagnosis from the resulting list.
The Family History Expanded Conditions template opens.
- On the Family History Expanded Conditions template, do the following:
- In Onset age, enter the family member's age at the time of diagnosis.
- If the family member died from the selected condition, select the Cause of death check box.
- To document any additional information, select the Comments link.
- Enter the details on the Family History Comments template, save, and close the template.
- To return to the Family Health History template, select Save to Grid & Close.
You can use Edit or Remove to manage the family history of the patient.
- Save and close the Family Health History template.
