You can add the family history of a patient from the Family category in the Family panel on the Histories template.
 
  - Open NextGen® Enterprise EHR.
 
  - Open the Histories template and then expand the Family panel.
 
  - In the Family panel, select Add.
   
    The 
Family Health History template opens. 
    
Note: NextGen® Enterprise EHR fills Specialty field with the current specialty. The available conditions for the specialty appear alongside a checkbox on the Family Health History template.
    
 
    
  - To indicate that the patient does not have any family history, select the No family history of checkbox.
 
  - In the Relationship field, select a description from the Family relation list.
 
  - In the Family member name field, enter the name of the selected family member, and do one of the following:
   
    - If the family member does not have any condition that can be reported, select the Alive and well option.
 
    - Select the conditions reported for the family member. 
     
      - If the family member has died due to the selected condition, select the Deceased option, and enter the age at death.
 
     
 
    - To add conditions that are not available on the template, select Other.
 
   
 
  - Select Save to Grid.
