You can document the single condition for a patient's family history.
 
  - Open NextGen® Enterprise EHR.
 
  - To open the Family Health History template and document family history details, do one of the following:
   
    - On the Histories template, expand the Family panel, and select Add.
 
    - In the History Summary panel on the Intake template, select Family, and select Add.
 
   
 
  - Select the check box next to an individual condition.
 
  - In the Onset age field, enter the family member's age at the time of diagnosis.
 
  - If the family member died from the condition, select the Cause of death check box.
   
    Note: When you select Cause of death, the Age at death field appears to enter the age of a dead person and application selects the deceased field automatically.
    
 
     
  - Select the Comments link to access the Family History Comments template. 
   
   
    - Enter any additional information about the family member and select Save & Close to close the Family History Comments template.
 
   
 
  - Select Save to Grid.
