Record a Barrier to Care

Open NextGen® Enterprise EHR. Select the Add button for the Barriers to Care panel.The Barriers to Care template opens. Barriers To Care Select the Onset Date field and select the date the barrier to care began and select the OK button.The selected date is populated into the Onset Date field. Type a meaningful description of the barrier to care in the Barriers to care field. If the barrier to care has been resolved, select the Resolved Date field and select the resolution date from the resulting calendar. The Status field is pre-filled with a status of Active. To specify another status, select the field and select the new status from the resulting pick list. If you have resolved the issue that presented the barrier to care, you can select a status of Deleted. The barrier is marked as deleted but the information remains in the system for tracking purposes. Optionally, type additional comments in the Comments field. To add another barrier to care, select the Save & Add New.A blank Barrie

Remove a Barrier to Care

Open NextGen® Enterprise EHR. Select to highlight the barrier to care on the Barriers to Care panel that you want to delete. Select the Remove button.You are prompted to confirm the delete. Select the Yes button.You are returned to the Care Coordination Home Page with the selected barrier to care removed from the Barriers to Care grid. Update a Referral

Open NextGen® Enterprise EHR. Select to highlight the referral on the Referral History grid that you want to update. Select Edit.The Referrals – Orders template appears. Referrals Order Update the template with the new information. Select Save.You are returned to the Care Coordination Home Page with the entry for the referral updated in the Referral History grid. Update an Agency or Team Member

Open NextGen® Enterprise EHR. Select to highlight the agency in the Agencies section or the team member in the Interdisciplinary Team section that you want to update. Select Edit for either section.The Care Coordination Team template appears populated with data for the selected agency or team member. Update the fields as required. You can change the information for either the agency or the team member or both, regardless of which Edit button you clicked. Select Save & Close.You are returned to your originating template with the team member or agency updated on the Agencies or Interdisciplinary team grid. Edit an Existing Nursing Care Plan

The existing nursing care plan information can be updated when required. Open NextGen® Enterprise EHR. Access the Care Coordination Home Template. Expand the Nursing Care Plan History panel. From the Nursing Care Plan History panel, select the record that you want to update. Select Edit.The Nursing Care Plan Data template opens. Update the information on the Nursing Care Plan Data template following the instructions given for adding a nursing care plan. To save the updates, select Save & Close. Enter, Review, and Update Guideline Status

You can document the details for the patient on the Care Guidelines grid view template. Open NextGen® Enterprise EHR. Select the Care Guidelines link on any of the main template. The Care Guidelines Paged View template opens. In the Care Guidelines panel, select the Grid View link. The Care Guidelines grid view template opens. Select a guideline item from the grid. Details for the care guideline fill in the fields. To edit the changes for information appeared for the guideline, do the following: In the Goal field, enter a goal for the patient or select from the list. Select the Status from the list. Select the Action from the list. Note: For guideline items that are ordered, fills in automatically. The Status and Action fields are read-only. Care Guidelines Select Update. The grid displays the updated Status. Note: To see the goal, select the item in the grid to display details in the fields.
