Obstetrics section The Parity detail auto calculation section is used to activate the automatic calculation of parity at the point of care when the pregnancy outcomes are entered. When the parity automatic calculations feature is turned off, the text "Parity auto-calculations not activated" appears. When you activate the Parity automatic calculate feature, the system adds a log in the OB Audit Log practice template. On the OB Audit Log practice template, you need to select the Parity detail auto calculation log type and the date range to view the items that have discrepancies in the parity numbers when compared to the pregnancy history grid entries. If the parity automatic calculation feature is turned on, the green text Auto-calculation appears on the Pregnancy Outcomes and OBGYN Synopsis templates. Enter a Historic Pregnancy in the Pregnancy Outcomes Template

You can enter a historic pregnancy in the Pregnancy Outcomes template. On the Patient Information Card in NextGen® Enterprise EHR, select OBGYN.The OBGYN Synopsis template opens. Select the Add/Update Detail button in the OBGYN Synopsis template.The Pregnancy Outcomes template opens. If you turn on the Parity automatic calculation setting, the Auto-calculation green text appears. Pregnancy Outcomes Select the Historic entry option in Pregnancy Entry Type field. Select the pregnancy number in the Pregnancy # field. Select a Full term or Premature pregnancy in the Gestational age section. For Full term pregnancy, if a pregnancy is >=37 weeks (37 weeks 0 days or later) the Full term field is incremented by 1. For Preterm pregnancy, if a pregnancy is <37 weeks (36 weeks 6 days or earlier) the Preterm field is incremented by 1 (Preterm = Premature) Note: Either a gestational age or the full term or preterm options must be documented for all outcomes with a gestational age >=20 weeks. Document the Pregnancy History

You can document pregnancy history information in the Pregnancy History panel. You can use the Pregnancy History panel to open the Pregnancy Outcomes template. Note: The Pregnancy Outcomes template is used during Prenatal and Postpartum visits as well as gynecology encounters. The workflow is different for each. In general, the most detail is entered during a Postpartum Visit. Pregnancy History Open the Histories tab. Open the Pregnancy History panel. Select the Add/Update button in the Pregnancy History panel.The Pregnancy Outcomes template opens. If you turn on the Parity auto-calculation setting, the Auto-calculation green text appears. Document the Pregnancy Outcomes Template

You can document information in the Pregnancy Outcomes template. The Pregnancy Outcomes template provides you the ability to view and modify a patient’s gravidity and parity. You can view and modify the total parity values with or without the details of each pregnancy. Minimal required fields are Pregnancy Entry Type, Pregnancy #, and Delivery Type. It is preferable, on an OB initial visit to use the gravidity/parity fields and Pregnancy Outcomes templates from within the OB templates. When you activate parity auto-calculation, parity automatically calculates the details when you document historical pregnancy or end a current pregnancy When you select End current pregnancy the Gravidity reflects a non-pregnant status. This option is used on a postpartum or post-abortion visit and not on an initial prenatal visit. Document Pregnancy Outcome in the OB Postpartum Template

You can document pregnancy outcome in the OB Postpartum template. Select the Postpartum tab. In the Pregnancy Outcome panel, select Add/Update.The Pregnancy Outcomes template opens. Enter the applicable information in the Pregnancy Outcomes template.The entered information displays in the Pregnancy Outcome grid. Verify the parity values are correct. Pregnancy Outcome grid Select the OB Problem List link.The OB Problem List template displays with the most recent pregnancy information in a new grid view. You cannot edit the information in the OB Problem List template. Select an entry in the Pregnancy Outcome grid. To generate the pregnancy outcome document , select the Pregnancy Outcome Document button. Note: The Details text box is read-only and no comments can be entered here. Document Pregnancy Details

You can document the pregnancy details for female patients as a part of medical history. On the Past Medical History template, in the Medical panel, click Add Pregnancy Detail. The Pregnancy Outcomes template opens. Pregnancy Outcomes template In the Quick Entry/Edit panel, in the Parity, Outcome Details, and Other sections, click the appropriate fields, and enter the values. In the Detailed Entry/Edit panel, enter the details of the pregnancy. Note: The fields that are marked with asterisk (*) are mandatory. Click Add To Grid to add the documented details to the Pregnancy history grid. Click Save & Close.
