Stage Cancer from the Onc Treatment Summary Template

The Onc Treatment Summary template provides the most comprehensive method of entering cancer staging and related data, including cancer medications, treatment notes and other data. For Cancer Registry Report, you must provide data on the following: Dx Date (Diagnosis Date) Cancer Type/Site Histology Behavior Laterality Body Site Diagnostic Confirmation Stage a Cancer Prior to staging cancer, you should enter the cancer diagnosis for the patient. If you do not, you can select a diagnosis during the procedure. Cancer Registry Reporting

During a patient encounter, the cancer registry functions enable you to document the diagnosis, evaluation, and treatment of active cancer for cancer registry reporting purposes. When the encounter is concluded and locked, the application generates a cancer report that is sent to the national cancer registry through an HL7 interface. By capturing these data elements, providers can meet the 2015 Quality Reporting certification objectives. To satisfy cancer registry reporting guidelines, you must record the following types of information. Patient demographic information In addition to the general patient data, the national cancer registry requires documentation of specific demographic data points, including race, ethnicity, occupation, and nation of birth. General cancer diagnosis and treatment data This includes data such as onset date, cancer notes, radiation notes, and problem description. Stage from the Urology Testicular Cancer Template

Open a patient encounter and select a Specialty of Urology and a Visit Type of Office visit. Navigate to either the Intake or the SOAP template. Scroll to the Reason for Visit panel. Click the drop-down arrow for the Other field in the left navigation panel. Select Testicular Cancer from the list. The Testicular Cancer (URO) template appears. Click Add/Update under the Cancer Staging grid. The Cancer Details template appears. Note: You can add and update records for the current encounter from this Staging grid, but you cannot remove records. In the Staging Detail panel, enter the details. Stage from the Urology Transitional Cancer Template

Open a patient chart and launch the Intake template. Select a Specialty of Urology and a Visit Type of Office visit. Navigate to either the Intake or the SOAP template. Scroll to the Reason for Visit panel. Click the drop-down arrow for the Other field in the left navigation panel. Select Urethral Cancer from the list. The Transitional Cancer (URO) template appears. Click Add/Update under the Cancer Staging grid. The Cancer Details template appears. In the Staging Detail panel, enter the details. Stage from the Dermatology Histories Template

Open a patient chart and launch the Intake template. Select a Specialty of Dermatology and a Visit Type of Office visit. Click the Histories tab to display the Histories template. Scroll to the Skin Cancer Log panel. Click Add under the Staging grid. The Cancer Details template appears. In the Staging Detail panel, enter the details. Stage from the Dermatology Skin Cancer History Log

Open a patient chart and launch the Intake template. Select a Specialty of Dermatology and a Visit Type of Office visit. Select Skin Cancer Hx from the Navigation menu. The Skin Cancer History Log template appears. Click Add/Update under the Staging grid. The Cancer Details template appears. Note: You can add and update records from this Staging grid, but you cannot remove records. In the Staging Detail panel, enter the details.
