Screening Tools Template Details
You can use the fields, checkboxes, and grids on the Screening Tools template to manage the screening tools.

| Screening Tools Available | List of screening tools supported by NextGen Healthcare and configured by the practice in alphabetical order. |
| Current Specialty | Displays screening tools configured for the selected specialty. When you select a specialty for an encounter, the Current Specialty option is selected by default and the screening tools configured for the selected specialty appears on the Screening Tools template. |
| All | Displays all screening tools. When you do not select a specialty for an encounter, the All option is selected by default and all the screening tools appear on the Screening Tool template. |
| Screening instrument | Name of the screening instrument. |
| Performed Date | Date when the screening is performed. |
| Score | Score associated with the screening tool. |
| Severity/Interpretation | Details about severity and interpretation. |
| Comments | Additional information you may want to enter. |
| Major Depressive Disorder (MDD) pre-treatment classification | Indicates MDD pre-treatment classification. |
| Plan | Description of the plan. |
| See scanned document | Names of scanned documents. |
| Follow Up | Follows-up plan of care. |
| Add | Adds a screening tool record. |
| Update | Updates a screening tool record. |
| Clear | Removes data entered on the template. |
| Screening Tools Performed | Historical list of screening tools documented for the patient. |
| Edit | Edits a record in the Screening Tools Performed grid. |
| Remove | Removes a record from the Screening Tools Performed grid. |
| Save & Close | Saves information entered and closes the template. |
| Cancel | Closes the template without saving information. |
| Health Promotion Plan | Documents BMI plan, depression plan, hypertension plan, pain management details, Fall Risk details, and Follow-Up Plans of Care details. |
| Care Plan | Defines and maintains a care plan for a patient, including health concerns, goals, interventions, and outcome. |
| A/P Details | Documents postoperative concerns and follow-up details. You can also enter any pertinent comments, differential diagnoses, plan, or provider details. |
| Referral | Applies provider signature and any updates to patient referrals. |
| My Phrases | Creates phrases. |
| Follow up | Documents information related to follow-up plan of care. |
| Billing Procedure | Adds a procedure code. |
| Modifier 1 | Adds a modifier from the list. |
| Modifier 2 | Adds another modifier from the list. |
| Diagnosis Code | Adds a diagnosis code from the list. |
| Submit to Superbill | Submits the bill. |
| Calculate | Calculates the score for some of the assessments such as PHQ9 and MDI 10 on the respective screening templates. |
| Q info icon | Provides additional information on the Screening Tools template. You can use the Screening Tools Log Configuration template to configure the Q info icon on the Screening Tools template. |
