Document Clinical Depression Screening
You can use the Patient Health Questionnaire (PHQ-2) button to document clinical depression screening and a follow-up plan. Open NextGen® Enterprise EHR. Open the Intake template. In the Review of Systems panel, select the PHQ-2 link.The Patient Health Questionnaire (PHQ-2) window opens. Select an option for each question. To open the Patient Health Questionnaire (PHQ-9) window, select the Patient Health Questionnaire (PHQ-9) link.The Patient Health Questionnaire PHQ-2 and PHQ-9 links appear in the Review of Systems panel based on the Show PHQ Link setting done on the Practice Configuration template. For more information about the Show PHQ Link setting, go to NextGen Healthcare Success Community, and download the Practice Configuration Template User Guide for NextGen® Adaptive Content Engine. Patient's Review of Systems
You can document the review findings for a patient after reviewing a list of systems. Review of Systems In Review of Systems, you can document symptoms for multiple systems on a single template with the use of one page ROS, or use the single systems in the left pane configured on the Framework Content system template. The patient health questionnaire PHQ-2 and PHQ-9 links appear on the section, based on the Show PHQ Link setting on the Practice Configuration practice template. Document the Patient's Suicide Risk Assessment
Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. Select Eval of Suicide Risk (BH), and then select OK. Select the program, specialty, and visit type. Note: Only programs that the patient is actively enrolled in appear in the list. You can select <No Program> to create an encounter without a program, and then add the program later. Document the reason for the suicide risk assessment. Document the patient's historical, clinical, and situational risk factors. Document the protective factors for the patient's mental health. Document Information with Other Screening Tools
You can document information using other screening tools that are not configured to open as websites or templates using the Screening Tools template. Open NextGen® Enterprise EHR. Select a patient encounter, and open a template. For example, Intake. Select Navigation, and then select Screening Tools. The Screening Tools template opens. To view the screening tools, do one of the following: To view the screening tools configured for the selected specialty, select Current Specialty. To view all the screening tools, select All. Note: When you do not select a specialty for an encounter, the All option is selected by default, and all the screening tools appear on theScreening Tools template. Select a screening tool that is not configured to open as a website or template. Enter the following information: Score Severity or interpretation Comments Major depressive disorder pre-treatment classification Select Add. Document Patient Screening
You can document patient screening in the Screening panel of the SBIRT Screening template. Open NextGen® Enterprise EHR. In the navigation bar, select Screening Tools. The Screening Tools template opens. In the Screening Tools Available section, select Screening, Brief Intervention, and Referral to Treatment (SBIRT). The SBIRT Screening template opens. SBIRT Screening Template Expand the Screening panel. Screening Template Enter the details related to screening instrument, score, severity or interpretation, comments, and Major Depressive Disorder (MDD). Performed Date is filled automatically. Select Add. The added records appear in the Screening Tools Performed grid. To access the AUDIT, AUDIT-C, CAGE, PHQ-2, and PHQ-9 screening tools, select the corresponding links. Enter an overall SBIRT impression or risk. NextGen® Enterprise EHR automatically fills Encounter for screening, unspecified and Z13.9 in the Select Assessment and Code fields respectively in the Brief Intervention panel. T
