Record the Risk Assessment on the CHA2DS2 VASc Screening Template

Cardiologists can record the CHA2DS2 VASc stroke risk assessment in Screening Tools.

1. Open NextGen® Enterprise EHR.
2. Select the Navigation bar.
3. Select Screening Tools.
   The Screening Tools template opens.
4. In the Screening Tools Available grid, select CHAD2DS2 VASc Screening.
   The CHA2DS2 VASc - Screening template opens.

   Note: In the Stroke Risk Assessment section, the application automatically selects age, gender, Hypertension, and Diabetes based on the patient's data.
5. Select the appropriate option Y or N for the following items.
   The application calculates the CHAD2DS2 VASc score and displays the recommended drug therapy with the alert icons.

   • Red icon - to indicate that the recommended drug therapy is not present in the Medication module.
   • Green icon - to indicate that the recommended drug therapy is active and present in the Medication module.
6. In the Other Data section, select the appropriate checkboxes.
7. Select Accept.
   The application adds an office services order for CHA2DS2 VASc Screening to the Orders module with a completed status.
8. Select Save & Close.
