Document Recent ER Visits (in last 12 months) Panel

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. If a patient has had any emergency room (ER) visits since his or her last visit or in the past 12 months, document those ER visits on this panel. Open NextGen® Enterprise EHR. On the Comp Assessment tab, expand Recent ER Visits (in last 12 months) panel. ER Visits Select the Location field and then select the hospital the patient was admitted to. Select the Date field and use the calendar to select the date the patient was admitted. Select the Diagnosis field and type a diagnosis code for the patient. Note: You can double-click the Diagnosis field to launch the Diagnosis search template. Use the template to search and select the diagnosis for the patient. Select the appropriate option for Summary report received. Select Add.The entered record appears in the Recent ER Visits panel. Remove an ER Visit Parent topic: Comp Assessment Tab

Record a Discharge Diagnosis

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. Open NextGen® Enterprise EHR. Select the Care Transitions tab.The Cm Care Transitions template opens. Cm Care Transitions On the General panel, select a blank Discharge diagnosis field.The Diagnosis Search window opens. Use the window to locate and select the appropriate diagnosis.The selected record appears in the Discharge Diagnosis field in the Discharge Diagnosis panel. Note: You can add up to four diagnoses. Document the Patient's Transfer Summary

You can document the patient's transfer details from one program to another. Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. Select Transfer Summary (BH), and then select OK. Select the program, specialty, and visit type. Select the patient's discharge date. To close the selected program for the patient, select the status of the patient's treatment in the program. In the Assessment/Diagnosis panel, document the patient's active diagnosis and assessment details. In the Transfer Summary panel, document the treatment details, post-transfer support details, risk details, and the reason for the patient's transfer. In the Transfer Plan panel, document the patient's active medications and transfer plans and instructions. Select Generate Document. Assessment and Diagnosis Functionality You can enter assessment information for the current encounter by using the Assessment/Diagnosis panel.

Document General Transition of Care Information for a Patient

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. Open NextGen® Enterprise EHR. Select the Care Transitions tab.The Cm Care Transitions template opens. Cm Care Transitions Select the PCP field.The field populates with the Primary Care Practitioner established in the patient's demographics. If there is no PCP selected in the patient chart, the Modify Patient Information template opens, enabling you to select a PCP. Do one of the following: Select the Encounter Type field to launch a list of transitional care encounter types and then select an encounter type from that list.
