Record Recent Patient Hospitalizations

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. Open NextGen® Enterprise EHR. On the Comp Assessment tab, expand Recent Hospitalizations panel. Select the Location field and then select the hospital the patient was admitted to from the resulting list. Select the Admit date field and then select the date the patient was admitted to the hospital from the resulting calendar. Select the D/C dates field and then select the date the patient was discharged from the hospital from the resulting calendar. Select the D/C diagnosis field, and then select a discharge diagnosis from the window that appears. Select the appropriate option for Summary report received. Select Add.The record appears in the Recent Hospitalizations. Repeat this procedure to add additional hospitalizations. Remove Patient Hospitalization Records Parent topic: Comp Assessment Tab

Comp Assessment Panels

The Comp (Comprehensive) Assessment template consists of the following panels: Panel Description General Panel The General panel displays the information documented in the Care Transition template. Recent ER Visits Panel Any recent emergency room visits by the patient are recorded on this panel. Recent Hospitalizations Panel If the patient had experienced any recent hospitalizations, then you would enter that information on this panel. Problem List Panel The Problem List panel displays a list of the patient's chronic and secondary problems as documented in the Problem List after Discharge panel on the Care Transition template. Active and Manage Problems The Active and Managed Problems panel is where active and managed problems are entered and monitored. Nursing Care Plan History The Nursing Care Plan History panel lists all the nursing care plans that have been created for a patient. Remove an ER Visit

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. Open NextGen® Enterprise EHR. On the Comp Assessment tab, expand Recent ER Visits (in last 12 months) panel. Highlight the record that you want to remove from the grid. Select Remove. Generate Care Management Documents

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. You can generate the following documents from the Document panel: Nursing Care Plan Summary Transition of Care Comprehensive Assessment Return Visit Care Management Summary Open NextGen® Enterprise EHR. On the Plan/Intervention tab, expand the Document panel. To add additional records, fill in the fields as appropriate and select Add. To generate a nursing care plan Summary document, select the Generate Nursing Care Plan Summary link. To generate a transition of care document, select the Generate Transition of Care link. To generate a comprehensive assessment document, select the Generate Comp Assess link. To generate a return visit document, select the Generate Return Visit link. Record a Discharge Diagnosis

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. Open NextGen® Enterprise EHR. Select the Care Transitions tab.The Cm Care Transitions template opens. Cm Care Transitions On the General panel, select a blank Discharge diagnosis field.The Diagnosis Search window opens. Use the window to locate and select the appropriate diagnosis.The selected record appears in the Discharge Diagnosis field in the Discharge Diagnosis panel. Note: You can add up to four diagnoses. Select the appropriate option for the Previous hospital admission in last 30 days field. Select the appropriate option for the Complete D/C summary received field. Parent topic: Care Transitions Tab

Use the Document Panel to Complete a Visit

Before you begin Ensure that you have selected the Specialty as Care Management and Visit Type as Case & Consent. Open NextGen® Enterprise EHR. On the Documentation template, expand the Document panel. Document Add or update the diagnosis code as needed. Choose the appropriate type of visit for the encounter. Select the Add button.
