Medication Reconciliation for a Patient

Medication reconciliation is a review of patient adherence to prescribed medications. Reconciliation can be performed by comparing two lists using the Electronic Reconciliation option or the Manual Reconciliation checkbox. This means you have a paper chart that you are comparing to the patient's current medication list, or you are documenting adherence to an existing medication list if there are no external sources to reconcile. Medication Review You can perform medication reconciliation in situations including, but not limited to, the following: New patients Periodically for established patients After a hospitalization or emergency department visit After care by another provider Whenever the provider thinks necessary The Medication Review panel contains two grids. The Medication List - includes a list of current medications that need to be reviewed for the patient adherence to the prescribed medication. Update the Adherence of a Medication

You can modify a patient's adherence to medication. Open NextGen® Enterprise EHR. Open the Intake template. Open the Medications Review template. Select a medication in the Medication Review grid.NextGen® Enterprise EHR fills Medication, Sig desc, and Adherence in respective fields below the Medication Review grid. Select Adherence, and select the comment from the list. Select Update.After you complete the medication reviews, you can select the Manual medication reconciliation completed checkbox to reconcile the medications, and enter any additional details in the Reconciliation Type panel. Review Medications Manually

You can manually review the medications details that are taken by a patient for adherence to the prescribed medicines. Open NextGen® Enterprise EHR. Open the Intake template. Open the Medications Review template. Select the Review – adherence checkbox. In the Review – adherence list, select a patient adherence comment, or enter the comment. To review all the medications in the Medication List, select the Review All – Taken As Directed button.The medications appear in the Medication Review grid below the Medication List grid. To review one medication at a time and select a different adherence for each, select one row at a time from the Medication List grid.Each selected medication moves to the Medication Review grid. To move a medication from the Medication Review grid, select Medication List, and select Remove. Review Medications Electronically

You can electronically review the medications details that are taken by a patient for adherence to the prescribed medicines. Open NextGen® Enterprise EHR. Open the Intake template. Open the Medications Review template. In Reconciliation Type, select Electronic Reconciliation. The Clinical Reconciliation module opens. Clinical Reconciliation On the Import tab, select the Surescripts® medication history file. Note: You must document the patient consent in the Medication module before importing a Surescripts® medication history file. Document the name of the person who confirmed the medication list. Perform reconciliation between the medications in the EHR panel and the Surescripts® file medications. Note: You can also complete the reconciliation from the Medication module using the Medical History and Reconcile buttons. This does not require Consolidated-Clinical Document Architecture (C-CDA). Document a Medication

You can add medications details for the treatment of a patient. Open NextGen® Enterprise EHR. Open the Intake or SOAP template. In the Medications panel, select Add /Update.The Medication Module window opens. Select Prescribe New.The Medication Search window opens. Search for a medicine from the list, and then select Select. Enter the start and stop date of the medication that the patient needs to take. Enter the quantity of the medication. To mention when and how the patient can take the medication, select Edit Sig:. Enter the number of refills allowed on the prescription of the patient. Enter the problem of the patient. If the medications are already prescribed, select the Prescribed elsewhere checkbox. Select Accept. Close the Medication module. In the Medications panel, to enter additional information about the medications, select the Comment link. Medications

You can document and manage the medication history and current medications of a patient. You can use the Medications panel to view the patient's current medications and medication history, enter additional prescriptions for the patient, and reconcile the medications listed with the actual active medications for the patient. You can use the No medications this encounter checkbox if there are no active medications for the patient. Medications Panel You can reconcile the medications for the patient using the Medications reconciled checkbox, and indicate that you have received the summary of care for the patient using the Summary of care received checkbox.
