Tobacco Cessation

You can use the Tobacco Cessation template to document the patient's tobacco use and the methods used for tobacco cessation. You can open the Tobacco Cessation template by using the Tobacco Cessation link. The Tobacco Cessation template enables you to document the tobacco history details, complete the medication screening questions, select the methods used for tobacco cessation, add assessments, and select and schedule the recommended treatments. If the patient has a medical condition due to tobacco use, then you can use the Tobacco Cessation template as a stand-alone visit for tobacco cessation. The Submit to Superbill button on the template enables you to submit the charges for the template. Note: For counseling sessions of less than 3 minutes, you can utilize the Risk Factors Config template to document the tobacco cessation discussed with the patient instead of the Tobacco Cessation template. The Finalize Template Workflow

You can do the following on the Finalize template: Submit and review the plan of care, based on your role, on the Provider Sign Off panel. Document Evaluation and Management (E&M) codes on the Evaluation and Management Coding panel. Calculate the patient's medical-decision-making level and submit the E&M codes to the Superbill module. Review the patient's history and physical examination details on the E&M Level panel and identify factors in the calculation of the E&M code under the 95 or 97 Coding Guidelines. Add Visit Code Modifiers

You can add or replace visit code modifiers. After you have calculated or selected a visit code on the Finalize (BH) template, the NextGen® Adaptive Content Engine may add modifiers to the visit code based on coding rules. You can also manually add up to two modifiers for each visit code and edit the modifiers. Open NextGen® Enterprise EHR. For a patient record, open a new encounter. Open the Finalize (BH) template. Expand the Evaluation and Management Coding panel. To add a modifier to a visit code, select Modifier(s) under the corresponding visit code, and then select a code from the list. To replace the modifier of a visit code, select a modifier, and then select another modifier from the list. Select SNOMED® CT Codes for Quality Reporting

You can use SNOMED® CT® codes for Quality Reporting. If you do not bill from NextGen® Enterprise EHR but need to meet the visit-code criteria for Clinical Quality Measures (CQMs), you can use a SNOMED® CT code instead of a CPT® code. Open NextGen® Enterprise EHR. For a patient record, open a new encounter. Open the Finalize template. On Evaluation and Management Coding, select the SNOMED Type (optional) link.The SNOMED Type template opens. Select a SNOMED® CT code. Select Save & Close.The NextGen® HQM application uses the selected code at the registry reporting time. Note: The code is not uploaded to the Procedures or Problems modules. History Details on Finalize

You can view the coding points assigned to each element of the patient's history component under the 95 or 97 Coding Guidelines in the History Details panel. The coding points are factored into the calculation of the Evaluation and Management (E&M) code under the 95 or 97 Coding Guidelines. If the 2021 Coding Guidelines are in effect, you can refer to this panel to review the history you documented; however, the history elements do not factor into the calculated E&M code. You can view the following information about history details.
